正常胎儿肺体积及发育的MRI评估

【摘要】:目的 探讨MRI成像技术在评估胎儿中晚期妊娠正常肺脏发育及成熟度的临床应用价值 材料与方法 回顾性分析本所自2005年12月份起至2013年12月份5892例行磁共振检查的中晚期妊娠(18至40周)的单活胎儿,筛选其中经超声和磁共振检查显示双侧胎儿肺脏发育正常且磁共振图像显示胎儿双侧肺脏清晰完整(以冠状位为主)的1286例胎儿,在SIMENSE磁共振后处理工作站,采用描点法测量胎儿肺脏每层截面积并结合扫描层厚层间距通过计算软件得出胎儿左肺体积、右肺体积及胎儿肺总体积数值,并选取感兴趣区分别测量胎儿肺脏及肝脏的信号强度,计算肺肝信号比,对数据进行统计,得到中晚期妊娠不同孕周正常胎儿左肺体积、右肺体积、肺总体积及肺肝信号比的正常值范围,并显示其与孕周的相关曲线。 结果 在中晚期妊娠正常胎儿中,随着孕周的增加胎儿右肺体积、左肺体积及肺总体积逐渐增加,呈线性正相关;而肺肝信号比则随着孕周的增加逐渐降低。相同孕周下胎儿右肺体积略大于左肺体积。 结论 磁共振检查不仅可以清晰准确显示胎儿肺脏的边缘及细微结构,而且可以量化评估肺脏的发育及成熟度,是胎儿呼吸系统检查的重要检查方法。
[Abstract]:Purpose To evaluate the clinical value of MRI imaging in evaluating the normal lung development and maturity of late fetal pregnancy Materials and methods A retrospective analysis of the single live foetus from December 2005 to December 2013 in 5892 cases of intermediate and late pregnancy (18 to 40 weeks) underwent magnetic resonance imaging (MRI). Among them, 1286 fetuses with normal development of bilateral fetal lungs and clear and complete bilateral lungs (mainly coronal position) were screened by ultrasonography and magnetic resonance imaging (MRI) postprocessing workstation. The cross sectional area of each layer of fetal lung was measured by drawing point method, and the values of left lung volume, right lung volume and total fetal lung volume were obtained by calculating software combined with scanning thick layer spacing. The signal intensity of fetal lung and liver were measured in the region of interest, and the signal ratio of lung and liver was calculated. The data were calculated, and the normal left lung volume and right lung volume of fetus were obtained in different gestational weeks of middle and late pregnancy. The normal range of lung volume and lung / liver signal ratio and the correlation curve between lung volume and gestational week were shown. Result In normal fetuses of middle and late pregnancy, with the increase of gestational weeks, the volume of left lung and the total volume of lung increased gradually, showing a linear positive correlation, while the ratio of lung and liver signal decreased with the increase of gestational week. The fetal right lung volume was slightly larger than the left lung volume at the same gestational week. Conclusion Magnetic resonance imaging not only can clearly and accurately display the edge and fine structure of fetal lung, but also can quantitatively evaluate the development and maturity of the lung. It is an important examination method for fetal respiratory system examination.
