三维超声评价健康生育年龄妇女的卵巢变化

【摘要】:目的: 1.探讨三维超声评估健康生育年龄妇女卵巢随年龄的变化。 2.探讨应用三维超声导出各年龄组健康生育年龄妇女的三维卵巢数据。 3.探讨二维超声与三维超声评估卵巢数据的相关性。 方法: 选取248名18-39岁月经周期规则、经期正常且月经为自发性,无妇产科疾患症状及不孕不育病史妇女,按年龄将受试者分为四个组,18-24岁,25-29岁,30-34岁,35-39岁。测量受试者的身高、体重、血压并在月经的第3-5天即卵泡早期用经阴道容积探头行超声检测,包括常规二维灰阶和彩色多普勒及三维灰阶和三维能量多普勒扫描。二维超声扫描子宫及双侧附件区排除妇产科疾病及卵巢黄体、卵巢囊肿等后测量双侧卵巢的体积、实时计算双侧卵巢窦卵泡的数量、最大卵泡的大小。二维完成后采集三维灰阶和能量多普勒数据,使用软件后处理并计算卵巢数据。包括最大卵泡的大小,总卵泡和卵巢的体积,窦卵泡的数量,卵巢的血管血流信息。卵巢的血管血流信息包括血管指数、血流指数及血管化血流指数。对上述数据进行统计学处理分析。 结果: 248人中有198人符合研究条件,被包括在最后的结果中。 四个年龄组间卵巢体积、窦卵泡的数量、总卵泡体积差异明显,有显著统计学意义(P0.001),双侧卵巢窦卵泡的数量、总卵泡体积、卵巢大小随年龄的增加而有显著的减少。双侧卵巢的最大卵泡的大小在各年龄组间差异没有统计学意义。双侧卵巢的三维血管血流数据在各个年龄组间差异没有统计学意义。二维实时测量的卵巢体积比三维VOCAL软件测量的卵巢体积小,三维软件SonoAVC自动计数的窦卵泡数量要少于二维窦卵泡的计数,二维最大卵泡测量比三维SonoAVC软件测量的数据要大。 18-24岁正常生育年龄妇女窦卵泡数量的Median (range)为23(10-58),25-29岁组的窦卵泡数量为21(11-50),30-34岁组的窦卵泡数量为18(5-38),35-39岁组的窦卵泡数量为12(2-20),窦卵泡数量在18-24岁组和25-29岁组间差异不明显(P=0.258)。18-24岁正常生育年龄妇女总卵巢体积的Median (range)为13.9(6.8-29.7)cm3,,25-29岁组总卵巢体积为13.7(6.3-29.3)cm3,30-34岁组的总卵巢体积为12.7(5.0-25.3)cm3,35-39岁组的总卵巢体积为10.4(4.2-23.6)cm3,25-29岁组和18-24岁组间卵巢体积数据差异没有统计学意义(P=0.440)。18-24岁正常生育年龄妇女总卵泡体积的Median (range)为2.05(0.32-3.81)cm3,25-29岁组总卵泡体积为1.93(0.29-3.69)cm3,30-34岁组的总卵泡体积为1.59(0.18-3.02)cm3,35-39岁组的总卵泡体积为1.28(0.12-2.30)cm3,25-29岁组和18-24岁组间窦卵泡体积数据差异没有统计学意义(P=0.553)。 结论: 健康无妇产科疾病症状生育年龄妇女的双侧卵巢体积、窦卵泡的计数、总卵泡体积值均随年龄的增加而呈明显下降趋势。各年龄组间双侧卵巢的三维血管血流指数差异没有统计学意义。
[Abstract]:Objective: 1. Objective: to evaluate the changes of ovary with age in healthy women of reproductive age by three-dimensional ultrasound. 2. Objective: to study the three-dimensional ovarian data of healthy women of childbearing age in different age groups by three-dimensional ultrasound. 3. To investigate the correlation between two-dimensional ultrasound and three-dimensional ultrasound in evaluating ovarian data. Methods: 248 women aged 18-39 years with normal menstrual cycle and spontaneous menstruation without symptoms of gynecologic diseases and infertility were divided into four groups according to their ages: 18-24 years old, 25-29 years old, 30-34 years old and 35-39 years old. The height, weight, blood pressure of the subjects were measured by transvaginal volume probe during the 3-5 days of menstruation, including routine two-dimensional gray scale and color Doppler and three-dimensional gray scale and three-dimensional power Doppler scanning. The volume of bilateral ovaries was measured after the uterus and bilateral adnexal region were excluded from obstetrical and gynecological diseases, ovarian corpus luteum and ovarian cyst. The number of antral follicles and the size of the largest follicles were calculated in real time. Three dimensional grayscale and energy Doppler data were collected after 2 D, and ovary data were processed and calculated by software. These include the size of the largest follicle, the volume of the total follicle and ovary, the number of antral follicles, and the blood flow information of the ovaries. The blood flow information of ovary includes vascular index, blood flow index and vascularized blood flow index. The above data were analyzed statistically. Results: Of the 248, 198 were eligible for the study and included in the final results. There were significant differences in ovarian volume, number of antral follicles and total follicle volume among the four age groups (P 0.001). The number of antral follicles, total follicular volume and ovarian size decreased with age. There was no significant difference in the size of the largest follicles in the bilateral ovaries between different age groups. The three-dimensional blood flow data of bilateral ovaries had no significant difference among different age groups. The volume of ovaries measured by two-dimensional real-time measurement was smaller than that measured by three-dimensional VOCAL software. The number of antral follicles measured automatically by SonoAVC software was less than that by two-dimensional antral follicles, and the measurement of two-dimensional largest follicles was larger than that measured by three-dimensional SonoAVC software. The number of antral follicles of 18-24 years old women with normal reproductive age was 2310 ~ (-58) / 25 ~ 29 years old. The number of antral follicles in the 30-34 age group was 185-38 / 35 ~ 39 years old, and the number of antral follicles was 12 ~ 2 ~ 20 ~ 20%. There was no significant difference in the number of antral follicles between 18-24 years old group and 25-29 years old group. 18-24 years old group had no significant difference in the number of antral follicles between 18-24 years old group and 25-29 years old group. The number of antral follicles in 30-34 years old group was 185-38%. The total ovarian volume of women aged 30 to 34 years old in normal reproductive age group was 12.7V 5.0-25.3cm ~ (35) ~ 39 years old, the total ovarian volume was 10.44.2-23.6 cm ~ (-3) cm ~ (29) and 18-24 years old, there was no significant difference in the total ovarian volume between the age group and the 18-24 age group, the total ovarian volume of the women of normal reproductive age was 13.9C 6.8-29.7g 路cm ~ (3) ~ (-1) ~ (25 ~ 29). There was no significant difference in the total ovarian volume between the age group and the 18-24 age group. The total follicle volume of normal fertile age women aged 18-24 years was 2.05 ~ 0.32-3.81g / cm ~ (3) and the total follicle volume of 25-29 years old group was 1.93 ~ (0.29-3.69) cm ~ (3) C ~ (-1) = 1.590.18-3.02cm ~ (335-39). There was no significant difference in total follicular volume between the age group (1.28) 0.12-2.30 cm ~ (325-29) and the group of 18-24 years old (P _ (0.553N). Conclusion: The bilateral ovarian volume, antral follicle count and total follicle volume of healthy women of childbearing age without symptoms of gynecology and obstetrics disease showed an obvious decreasing trend with the increase of age. There was no significant difference in three-dimensional vascular blood flow index between different age groups.
