妊娠糖尿病患者脐血炎症因子水平及其相关性研究

【摘要】:背景:妊娠糖尿病(gestational diabetes mellitus, GDM)患者子代代谢综合征(metabolic syndrome, MetS)发病风险增加。高敏C反应蛋白(high sensitivity C-reactive protein, hs-CRP)及白细胞介素6(interleukin-6, IL-6)是常见的炎症因子,与代谢性疾病的发生密切相关。但目前关于GDM患者脐血hs-CRP、IL-6水平及其与子代营养状态及对胰岛素的敏感性的相关性研究仍很少。 目的:探讨GDM患者脐血hs-CRP、 IL-6水平及其与子代营养状态及对胰岛素的敏感性的相关性。 方法:2009年-2011年于我院行剖宫产孕妇,,其中26名为GDM患者,28名为糖耐量正常(normal glucose tolerance, NGT)者。测定脐血hs-CRP、IL-6、血糖及胰岛素水平,使用Ponderal指数(ponderal index,PI)评估子代营养状态,胰岛素稳态模型(homeostasis model assessmentof insulin resistance, HOMA-IR)评估对胰岛素的敏感性。 结果:GDM组HOMA-IR、胰岛素水平、PI均较对照组显著增加。两组脐血hs-CRP水平及IL-6水平无显著统计学差异。HOMA-IR及脐血胰岛素均与子代出生体重及PI正相关。NGT组脐血IL-6水平与子代出生体重负相关,与hs-CRP正相关,但校正孕妇年龄、孕前体重指数(body mass index, BMI)、胎龄及子代性别后上述相关性不再成立。GDM组脐血hs-CRP、IL-6水平与子代营养状态及胰岛素敏感性无相关性。 结论:1. GDM患者子代出生时即存在较NGT者子代高的胰岛素水平,且对胰岛素的敏感性下降。 2.脐血胰岛素水平及对胰岛素的敏感性与子代营养状态密切相关。 3. GDM患者及NGT者脐血hs-CRP及IL-6水平与子代营养状态及对胰岛素的敏感性均无显著相关性。
[Abstract]:Background: metabolic syndromes (MetSs) in offspring of gestational diabetes mellitus, GDM) patients with gestational diabetes mellitus are at increased risk. Gao Min C-reactive protein high sensitivity C-reactive protein (hs-CRP) and interleukin-6 (IL-6) are common inflammatory factors, which are closely related to the occurrence of metabolic diseases. However, there are few studies on the relationship between the level of hs-CRPU IL-6 in umbilical cord blood and the nutritional status of offspring and insulin sensitivity in patients with GDM. Objective: to investigate the relationship between the levels of hs-CRP, IL-6 in umbilical cord blood and the nutritional status of offspring and insulin sensitivity in patients with GDM. Methods: cesarean section was performed in our hospital from 2009 to 2011, of which 26 were GDM patients and 28 were normal glucose tolerance, NGT) patients with normal glucose tolerance. The levels of hs-CRP IL-6, blood glucose and insulin in umbilical cord blood were measured. The nutritional status of offspring was evaluated by Ponderal index. The homeostasis model assessmentof insulin resistance, HOMA-IRR was used to evaluate the sensitivity of insulin to insulin. Results HOMA-IRand insulin level and Pi were significantly increased in the weight GDM group compared with the control group. There was no significant difference in hs-CRP and IL-6 levels between the two groups. HOMA-IR and insulin were positively correlated with birth weight and Pi. The IL-6 level of cord blood in NGT group was negatively correlated with birth weight of offspring, and positively correlated with hs-CRP, but the age of pregnant women was adjusted. Body mass index (BMI), gestational age and sex of offspring were no longer correlated with the level of hs-CRPU IL-6 in cord blood of GDM group. There was no correlation between hs-CRPU IL-6 level and nutritional status and insulin sensitivity of offspring in GDM group. Conclusion 1. There were higher insulin levels and lower insulin sensitivity in GDM offspring than in NGT offspring. 2. Cord blood insulin level and insulin sensitivity are closely related to the nutritional status of the offspring. 3. There was no significant correlation between the levels of hs-CRP and IL-6 in cord blood of GDM patients and NGT patients and the nutritional status and insulin sensitivity of their offspring.
