妇科肿瘤患者医院感染特征及相关易感因素分析

【摘要】:目的分析妇科肿瘤患者医院感染特征及相关易感因素,为临床感染防控提供指导。方法收集564例妇科肿瘤患者临床资料,采用全自动细菌微生物鉴定仪鉴定病原菌。采用K-B纸片扩散法检测病原菌的耐药情况,分析患者感染的易感因素。结果 564份妇科肿瘤患者各类送检标本中分离出病原菌170株(30.14%);181份生殖道标本分离出93株,164份泌尿道标本分离出41株,117份呼吸道标本分离出25株,102份口腔标本分离出11株,分离率分别为51.38%、25.00%、21.37%和10.78%;170株病原菌中革兰阴性菌105株,革兰阳性菌50株,真菌15株。革兰阴性菌中大肠埃希菌53株,肺炎克雷伯菌24株,其他革兰阴性菌28株;革兰阳性菌中表皮葡萄球菌21株,金黄色葡萄球菌14株,其他革兰阳性菌15株;真菌中白色假丝酵母菌11株,近平滑假丝酵母菌4株。大肠埃希菌对头孢唑啉、环丙沙星、庆大霉素、万古霉素、美罗培南的耐药率分别为100.00%、7.55%、52.83%、0.00%和0.00%;表皮葡萄球菌对头孢唑啉、环丙沙星、庆大霉素、万古霉素、美罗培南的耐药率分别为28.57%、76.19%、52.38%、0.00%和23.81%。年龄≥60岁、接受化疗、合并慢性疾病、使用抗菌药物、住院时间≥1个月均为妇科肿瘤患者医院感染易感因素。结论妇科肿瘤患者感染病原菌类型以大肠埃希菌和表皮葡萄球菌为主,生殖道是病原菌主要感染部位。大肠埃希菌感染可选用美罗培南,表皮葡萄球菌感染可选用万古霉素进行治疗。
[Abstract]:Objective to analyze the characteristics of nosocomial infection and related susceptible factors in gynecological tumor patients, and to provide guidance for clinical infection prevention and control. Methods the clinical data of 564 patients with gynecological tumors were collected. K-B disk diffusion method was used to detect the drug resistance of pathogenic bacteria and the susceptible factors of infection were analyzed. Results 170 strains of pathogenic bacteria were isolated from 181 genitourinary tract specimens of 564 gynecological tumor patients, and 93 strains of urethra were isolated from 164 specimens of urinary tract, 41 strains from 117 respiratory tract specimens, 25 strains from 102 oral specimens, and 11 strains from oral cavity. The isolation rates were 21.37% and 10.78%, respectively, of which 105 were gram-negative bacteria, 50 Gram-positive bacteria and 15 fungi. Among them, 53 were Escherichia coli, 24 were Klebsiella pneumoniae, 28 were other gram-negative bacteria, 21 were Staphylococcus epidermidis, 14 were Staphylococcus aureus, and 15 were Gram-positive bacteria. There were 11 strains of Candida albicans and 4 strains of Candida albicans. The resistance rates of Escherichia coli to cefazolin, ciprofloxacin, gentamicin, vancomycin and meropenem were 100.007.5552.83% and 0.00000%, respectively, while Staphylococcus epidermidis was resistant to cefazolin, ciprofloxacin, gentamycin, vancomycin, and vancomycin. The drug resistance rate of meropenem was 28.57 and 76.19%, respectively, and that of meropenem was 0.00% and 23.81%, respectively.
