【摘要】：目的：探讨妊娠期糖尿病(gestational diabetes mellilus, GDM)患者妊娠晚期血糖、血脂水平变化与新生儿体重的关系。方法：选择2012年5月至2013年6月在新疆医科大学第一附属医院严格检查并分娩,且临床资料完整的妊娠期糖尿病孕妇共150例,其中汉族107例,维族43例,并以同期120例正常妊娠期妇女,其中包括汉族97例,维族23例作为对照组,并采用酶学的方法来测定妊娠期糖尿病组孕妇和正常组孕妇妊娠晚期血糖、血脂的水平变化,同时测定两组孕妇分娩后的新生儿体重。结果：1.与正常组相比,妊娠期糖尿病孕妇的总胆固醇(TC)水平、低密度脂蛋白(LDL)胆固醇水平及高密度脂蛋白(HDL)胆固醇水平均高于正常组,但差异无明显统计学意义(P0.05),而空腹血糖及甘油三酯(TG)水平显著高于正常组(P0.01)。2.维族孕妇比汉族孕妇血糖偏高,有统计学意义(P0.01),血脂水平偏高,无统计学意义。3.Person's分析显示,妊娠期糖尿病孕妇妊娠晚期血糖、甘油三酯水平与新生儿体重存在显著的正相关性(P0.01)。结论：妊娠期糖尿病孕妇妊娠晚期血清血糖、甘油三酯水平的增加与新生儿体重有着密切的相关性。妊娠晚期孕妇血清甘油三酯水平可能是妊娠期糖尿病孕妇分娩巨大儿的独立预测因素。妊娠期糖尿病患者不同孕期血脂变化不同,不同糖代谢异常患者血脂变化不同。
[Abstract]:Objective: to investigate the relationship between the changes of blood glucose, blood lipids and neonatal weight in gestational diabetes mellilus, GDM) patients with gestational diabetes mellitus. Methods: a total of 150 pregnant women with gestational diabetes mellitus (GDM) were selected from May 2012 to June 2013 in the first affiliated Hospital of Xinjiang Medical University, including 107 cases of Han nationality and 43 cases of Uygur nationality. In the same period, 120 normal pregnant women, including 97 Han women and 23 Uygur women, were used as control group. The changes of blood glucose and blood lipid levels in pregnant women with gestational diabetes mellitus (GDM) and normal pregnant women in late pregnancy were measured by enzymatic method. At the same time, the birth weight of the two groups of pregnant women after delivery was measured. The result is 1: 1. Compared with the normal group, the levels of total cholesterol (TC), low density lipoprotein (LDLL) cholesterol and high density lipoprotein (HDL) cholesterol in pregnant women with gestational diabetes mellitus were higher than those in normal women. However, there was no significant difference in the levels of fasting blood glucose (FBG) and triglyceride (TGG) between the two groups. The blood glucose of Uygur pregnant women was higher than that of Han women (P 0.01), and the level of blood lipid was higher than that of Han women. There was no significant difference in blood glucose, triglyceride levels and neonatal weight in pregnant women with gestational diabetes mellitus (GDM). Conclusion: the increase of serum glucose and triglyceride levels in pregnant women with gestational diabetes mellitus has a close correlation with neonatal weight. Serum triglyceride level may be an independent predictor of macrosomia in pregnant women with gestational diabetes mellitus. The changes of serum lipids were different in gestational diabetes mellitus (GDM) and abnormal glucose metabolism (GDM).
