联合检测大脑中动脉及脐动脉阻力指标对胎儿宫内缺氧的诊断价值

【摘要】:目的探讨彩色多普勒超声检测大脑中动脉(MCA)和脐动脉(UA)阻力指标对胎儿宫内缺氧的诊断价值。方法选取该院临床确诊的胎儿宫内缺氧孕妇121例,正常单胎无并发症孕妇200例,利用彩色多普勒超声检查胎儿MCA及UA阻力指标,比较分析其敏感性、准确性。结果与对照组比较,缺氧组胎儿MCA各阻力指标均明显降低,而UA各阻力指标显著升高,差异均有统计学意义(P0.05);缺氧组MCA与UA各阻力指标比值较对照组均明显降低,差异有统计学意义(P0.01);MCA联合UA评估胎儿宫内缺氧灵敏度、准确率高,漏诊率低,而单以MCA较UA评估胎儿宫内缺氧灵敏度、准确率高,漏诊率低。结论联合检测MCA及UA阻力指标能够较准确预测胎儿宫内缺氧,且较单项指标灵敏度高、准确率高,漏诊率低。
[Abstract]:Objective to evaluate the diagnostic value of color Doppler ultrasonography in the diagnosis of fetal intrauterine hypoxia by detecting the resistance of middle cerebral artery (MCA) and umbilical artery (UA). Methods 121 cases of fetal intrauterine hypoxia and 200 cases of normal single fetus without complications were selected. The resistance indexes of fetal MCA and UA were examined by color Doppler ultrasound, and their sensitivity and accuracy were compared and analyzed. Results compared with the control group, the resistance index of MCA in hypoxia group decreased significantly, while the resistance index of UA increased significantly (P0.05), the ratio of resistance index of MCA to UA in hypoxia group was significantly lower than that in control group. The difference was statistically significant (P0.01). The sensitivity of intrauterine anoxia was higher and the rate of missed diagnosis was lower than that of MCA alone. The accuracy rate and missed diagnosis rate of MCA alone were higher than those of UA. Conclusion combined detection of MCA and UA resistance indexes can accurately predict fetal intrauterine hypoxia, and has higher sensitivity, higher accuracy and lower missed diagnosis rate than single index.
