孕晚期睡眠困扰与心血管代谢风险的关联

【摘要】:目的:探讨孕晚期睡眠困扰与心血管代谢风险因子即血压(BP)、体质数(BMI)间的关系。方法:采用失眠严重指数量表(ISI),Epworth嗜睡量表(ESS),匹兹堡睡眠质量指数(PSQI)评估妊娠第三期孕妇的睡眠质量;采集血压,体重,身高数据;分析失眠严重程度,日间嗜睡程度及睡眠效率(SE)与血压及BMI间的关系。结果:ISI与血压、孕前BMI、目前BMI均无明显相关;ESS与收缩压(SBP)(r=-0.13,P0.05)及舒张压(DBP)(r=-0.11,P0.05)有明显负相关,孕周与BMI后(r=0.20,P0.01)及舒张压(r=0.21,P0.01)呈显著正相关;年龄与孕前后BMI(r=0.21,r=0.14,P0.01)及收缩压(r=0.12,P0.01)均呈显著正相关。在分组比较中,ISI总分≥8分组ESS得分明显更高(t=-3.2,P0.01),SE更低(t=5.2,P0.01),血压和BMI两组间无明显差异;ESS总分10分组,SBP(t=3.2,P0.01)及DBP(t=2.5,P0.01)均明显更低,ISI明显更高(t=-2.9,P0.01)。结论:孕妇失眠与日间嗜睡相互影响,失眠对血压的影响尚不能定论,日间嗜睡可能会降低血压升高的风险,同时日间睡眠过多也易导致失眠的持续或加重,所以对失眠的干预需衡量日间嗜睡的利弊。
[Abstract]:Objective: to investigate the relationship between sleep disturbance in late pregnancy and cardiovascular metabolic risk factors, I. E. blood pressure (BP), body mass (BMI). Methods: sleep quality of pregnant women in the third trimester of pregnancy was evaluated by Epworth sleepiness scale (ESS) and Pittsburgh sleep quality index (PSQI), blood pressure, weight and height were collected, and the severity of insomnia was analyzed. Association of daytime sleepiness and sleep efficiency (SE) with blood pressure and BMI. Results there was no significant correlation between ESS and systolic blood pressure (SBP), diastolic blood pressure (DBP) and diastolic blood pressure (DBP) (r-0.11P 0.05). There was a significant positive correlation between gestational weeks and BMI (r 0.20 P 0.01) and diastolic blood pressure (r 0.21 P 0.01), age was positively correlated with BMI before pregnancy (r 0.21) and systolic blood pressure (r 0.12 P 0.01).
