腹腔镜辅助经脐入路治疗小儿卵巢囊肿蒂扭转6例报告

【摘要】：目的探讨腹腔镜辅助经脐入路治疗小儿卵巢囊肿蒂扭转的临床价值。方法 2012年3月~2015年3月对6例小儿卵巢囊肿蒂扭转行急诊腹腔镜辅助经脐入路探查、卵巢囊肿切除术,术中行囊液穿刺减压后,经脐扩大切口将囊肿提出腹腔外行囊肿剥除术。结果 6例均顺利完成手术,成功保留患侧卵巢,手术时间45~90 min,(50±22)min;术后住院3~5 d,平均4 d。6例随访6~12个月,平均10个月,腹部B超检查无复发,切口瘢痕小,脐部外观满意。结论腹腔镜辅助经脐入路手术具有创伤小、恢复快、操作简单、易推广等优点,对于腹腔镜手术操作困难的患者是一种较好选择。
[Abstract]:Objective to evaluate the clinical value of laparoscopic assisted umbilical approach in the treatment of ovarian cyst pedicle torsion in children. Methods from March 2012 to March 2015, 6 children with ovarian cyst pedicle torsion underwent laparoscopic assisted transumbilical approach exploration, ovarian cyst resection, and puncture and decompression of cystic fluid during the operation. Extraperitoneal excision of cysts was performed through an enlarged incision of the umbilical cord. Results the operation was successfully completed in all 6 cases and the affected ovary was preserved successfully. The operative time was 4590 minutes (50 卤22) mins, and the postoperative hospitalization was 3 ~ 5 days, with an average of 4 days. 6 cases were followed up for 6 ~ 12 months (mean 10 months). No recurrence was found in abdominal B-ultrasound examination, the incision scar was small, and the appearance of umbilical cord was satisfactory. Conclusion Laparoscopic assisted transumbilical approach has the advantages of less trauma, quick recovery, simple operation and easy to be popularized. It is a good choice for patients with difficult operation of laparoscopic surgery.
