胎盘中IGF-1、IGF-2、IGF-1R以及IGFBP-3表达水平与妊娠期糖尿病巨大儿的相关性研究

【摘要】:目的:分别测定分娩巨大儿的妊娠期糖尿病(GDM)孕妇的胎盘组织和分娩巨大儿的正常孕妇胎盘组织中胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子-2(IGF-2)、胰岛素样生长因子-1受体(IGF-1R)及胰岛素样生长因子结合蛋白-3(IGFBP-3)的mRNA表达水平,探讨IGF-1、IGF-2、IGF-1R及IGFBP-3在妊娠期糖尿病巨大儿形成中的作用,为研究妊娠期糖尿病巨大儿发病机制提供理论依据,同时也为临床妊娠期糖尿病的治疗和预防巨大儿的产生提供理论支持。 资料与方法: 1、资料来源 1.1GDM组:选择2010年1月至2012年12月在中国人民解放军306医院产前检查、孕期确诊妊娠期糖尿病且分娩巨大儿的初产妇胎盘22例,,病例入选条件:(1)孕母既往无糖尿病史,于孕24-28周行75gOGTT试验,结果判定参照2011年美国糖尿病学会(ADA)诊断标准,血糖水平满足以下之一者:空腹血糖≥5.1mmol/l,服糖后1小时血糖≥10.0mmol/l,服糖后2小时血糖≥8.5mmol/l,均为首次妊娠,无妊娠期高血压病、妊娠期甲状腺功能异常等其他妊娠期并发症,同时无家族遗传病史(2)新生儿出生体重≥4000g,且无先天畸形,Apgar评分均为7-10分。 1.2正常对照组:选择同期在本院住院并分娩、无妊娠期糖尿病且分娩巨大儿的初产妇胎盘组织18例,病例入选条件:(1)所有产妇均为初次妊娠,既往无孕产史,孕期糖耐量试验正常,无其他任何妊娠期合并症,且无家族性遗传性疾病;(2)新生儿无先天畸形,出生体重≥4000g之间,Apgar评分均为7-10分。 2、方法 2.1标本采集及处理符合上述条件的孕妇在胎儿娩出后,均在分娩或剖宫产时于胎盘娩出后5分钟内从胎盘母体面中央(避开坏死及钙化区)取绒毛组织块约1cm×1cm×1cm,生理盐水溶液充分漂洗后置入冻存管封存,液氮中保存以待mRNA的提取。 2.2胎盘组织中胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子2(IGF-2)、胰岛素样生长因子-1受体(IGF-1R)及胰岛素样生长因子结合蛋白-3(IGFBP-3)mRNA检测采用Real Time PCR技术测定妊娠期糖尿病并分娩巨大儿产妇胎盘组织及正常孕妇分娩巨大儿胎盘组织中胰岛素样生长因子受体I(IGF-1R)以及IGF结合蛋白3(IGFBP-3)的mRNA水平。 结果 1、GDM组患者胎盘中IGF-1、IGF-2、IGF-1R的表达量要显著高于正常对照组,差异有统计学意义(P0.05)。 2、在妊娠糖尿病患者胎盘中IGFBP-3的表达水平较正常对照组低,差异有统计学意义(P0.05)。 结论 1、提示GDM孕妇巨大儿的分娩可能与胎盘组织中IGF-1、IGF-2及IGF-1R表达水平的升高有密切关系; 2、GDM组胎盘组织中IGFBP-3表达水平的降低可能是导致GDM孕妇巨大儿发生的重要原因之一。

The expression of mRNA in receptor (IGF-1R) and insulin-like growth factor binding protein 3 (IGFBP-3), To explore the role of IGF-1R and IGFBP-3 in the formation of gestational diabetic macrosomia, to provide theoretical basis for studying the pathogenesis of gestational diabetic macrosomia, and to provide theoretical support for clinical treatment of gestational diabetes mellitus and prevention of macrosomia.
