新辅助化疗对宫颈鳞癌maspin表达及肿瘤微淋巴管生成影响

【摘要】:目的大量研究提示,maspin在诱导肿瘤细胞凋亡、抗肿瘤血管生成等方面功能显著,可能是一个功能全面的抑癌基因。本研究观察抑癌基因maspin及肿瘤微淋巴管(tumor lymphangiogenesis,TLM)在宫颈鳞癌新辅助化疗(neoadjuvant chemotherapy,NACT)前后表达的变化,探讨maspin在抑制宫颈鳞癌进展中与肿瘤微淋巴系统之间的关系。方法收集2008-07-01-2015-03-31滨州医学院附属医院妇科接受宫颈活检及化疗后手术的患者的存档蜡块,从中选取55例ⅠB_2~ⅡA期宫颈鳞癌患者,分别收集其化疗(TP化疗方案)前后标本,并相应取出对应备用的组织,采用免疫组化法及RT-PCR技术检测标本中maspin蛋白及maspin mRNA的表达水平,采用免疫组化法检测TLM的生成情况,分析NACT后maspin及TLM之间的关系,并探讨NACT治疗宫颈鳞癌的临床疗效与年龄、FIGO分期的关系及分析不同临床疗效组NACT前后宫颈癌组织中maspin的表达情况。结果化疗前maspin阳性表达率为36.36%,化疗后maspin阳性表达率为83.64%,化疗后较化疗前maspin表达反应性上调,χ~2=25.606,r=0.482,P0.01;ⅠB_2期或ⅡA期宫颈癌患者NACT后maspin mRNA表达水平较NACT前均高,差异均有统计学意义,P0.05;ⅠB_2期与ⅡA期宫颈癌患者化疗后maspin mRNA之间差异无统计学意义,P=1.36;化疗后微淋巴管密度(microlymphatic vessel density,LMVD)为4.56±2.21,较化疗前的8.19±4.17显著降低,P=0.021;NACT后,宫颈鳞癌组织maspin蛋白(r=-0.65,P=0.024)或mRNA(r=-0.57,P=0.031)表达上升及LMVD下降之间呈显著负相关;NACT治疗宫颈鳞癌的临床总有效率达81.82%,不同临床疗效组中,临床部分缓解组(PR)NACT后maspin阳性率较NACT前相比显著增加,差异有统计学意义,P0.05;CR组及SD组NACT后maspin的阳性表达率较NACT前相比也有所增高,差异无统计学意义,P0.05;NACT疗效与患者年龄及分期差异无统计学意义,P0.05。结论 NACT疗效可能与年龄、FIGO分期无关。maspin与宫颈癌NACT化疗疗效关系密切,可作为疗效敏感性指标;上调maspin的表达可能会抑制肿瘤微淋巴系统生成,从而抑制肿瘤细胞生长、侵袭及转移,这可能是新辅助化疗抑制宫颈癌进展的机制之一。
