101例卵巢恶性性索间质肿瘤临床病理分析

【摘要】：目的：通过复习卵巢恶性性索间质肿瘤的临床病理特征探讨其诊治策略。 方法：回顾性收集浙江大学医学院附属妇产科医院1998年5月-2013年8月收治的101例卵巢恶性性索间质肿瘤患者的临床病理资料及预后情况。 结果：卵巢恶性性索间质肿瘤的发病年龄为8-78岁,中位年龄47.50岁。49.5%患者因阴道不规则出血就诊,其次为发现盆腔包块、腹痛、闭经、不孕。24例(23.76%)患者行保留生育功能手术,其中13例为Ia期,11例为Ic期。根据术后病理结果Ⅰ期患者有99例(98.02%),Ⅱ期和Ⅲ期均1例(0.99%)。所有淋巴结切除者均未见转移。26.67%患者合并子宫内膜病变,其中4%为子宫内膜癌。术后44例患者辅助化疗,随访的69例患者均存活,其中1例患者复发后行肿瘤减灭术。 结论：卵巢恶性性索间质肿瘤多发生于围绝经期,发现时多为Ⅰ期,绝大多数为单纯或混合颗粒细胞瘤,预后良好。Ⅰ期低危患者可不行淋巴结切除,年轻患者可保留生育功能手术,但建议行子宫内膜诊刮或活检。
[Abstract]:Objective: To explore the diagnosis and treatment strategy of ovarian malignant stromal tumors by reviewing the clinicopathological features.
Methods: the clinicopathological data and prognosis of 101 cases of ovarian malignant stromal tumors admitted in the Department of Obstetrics and Gynecology, affiliated to the Medical College of Zhejiang University, in August -2013 May 1998, were retrospectively reviewed.
Results: the age of the malignant stromal tumor of the ovary was 8-78 years old. The median age of 47.50 years old.49.5% patients was diagnosed with irregular vaginal bleeding. The second was to find pelvic mass, abdominal pain, amenorrhea, and infertility (23.76%) of the.24 patients (23.76%), of which 13 were Ia and 11 were Ic. According to the postoperative pathological results, there were 99 patients. Cases (98.02%), stage II and stage III were all 1 cases (0.99%). None of the patients with metastatic.26.67% had endometrial lesions, of which 4% were endometrial carcinoma. 44 patients received adjuvant chemotherapy after operation, and 69 patients were all survived, 1 of whom underwent swelling and subtraction after recurrence.
Conclusion: ovarian malignant stromal tumors mostly occur in perimenopausal period, most of which are stage I, and most of them are simple or mixed granulosa cell tumor, and the prognosis is good.
