腹腔镜下腹股沟淋巴清扫术治疗外阴癌的临床研究

【摘要】：目的探讨腹腔镜下腹股沟淋巴清扫术治疗外阴癌的临床效果。方法将外阴癌患者56例根据术式分为2组,对照组27例患者采用改良外阴广泛切除术联合腹股沟淋巴结切除术治疗,观察组29例患者采用腹腔镜下腹股沟淋巴清扫术治疗,术后随访1 a,比较2组患者的手术指征、手术疗效、并发症情况、术后1 a生存情况及复发情况。结果观察组患者手术时间、术中出血量、术后拔管时间、切口愈合时间、术后住院时间均明显小于对照组,淋巴结切除数明显多于对照组,并发症发生率明显低于对照组,差异均有统计学意义(P均0.05)。2组术后1 a生存率、术后1 a复发率比较均无统计学差异(P均0.05)。结论腹腔镜下腹股沟淋巴清扫术是治疗外阴癌的有效方法,对患者造成的创伤较小,利于患者的术后恢复,可明显缩短患者的治疗时间,治疗更为彻底,引发的并发症少,安全性更高,值得临床推广使用。
[Abstract]:Objective to investigate the clinical effect of laparoscopic inguinal dissection for vulvar carcinoma. Methods 56 patients with vulvar carcinoma were divided into two groups according to the operation method. 27 patients in the control group were treated with modified vulvar extensive excision combined with inguinal lymph node resection, and 29 patients in the observation group were treated with laparoscopic inguinal lymph dissection. All patients were followed up for 1 year. The indications, curative effects, complications, survival and recurrence of the two groups were compared 1 year after operation. Results the operative time, blood loss, extubation time, incision healing time, postoperative hospitalization time, lymph node excision rate and complication rate in the observation group were significantly lower than those in the control group, and the incidence of complications in the observation group was significantly lower than that in the control group. There was no significant difference in 1 year survival rate and 1 year recurrence rate between the two groups (P 0.05). Conclusion Laparoscopic inguinal lymph dissection is an effective method for the treatment of vulvar carcinoma. It is more safe and worthy of clinical application.
