坤泰胶囊对在体外受精-胚胎移植中卵巢低反应患者的影响

【摘要】：目的探讨坤泰胶囊治疗在体外受精-胚胎移植(in vitrofertilization-embryo transfer,IVF-ET)中卵巢低反应(poor ovarian response,POR)患者的临床效果。方法 90例均为2015年4月—2016年4月在山东省潍坊市人民医院生殖医学科就诊的卵巢储备功能减退(diminished ovarian reserve,DOR)的不孕症患者。均采用常规微刺激方案,充分知情同意,根据是否同意用药分观察组(48例)和对照组(42例),观察组进入周期前1个月开始口服坤泰胶囊;对照组直接进入周期。观察两组患者年龄、不孕年限、体质量指数(body mass index,BMI)、肾阴虚证候评分、基础卵泡刺激素(basal follicle stimulating hormone,bFSH)、基础促黄体生成素(basal luteinizing hormone,bLH)、基础雌二醇(basal estradiol,bE2)、基础窦卵泡数(antral follicle count,AFC)、抗苗勒管激素(anti-Müllerian hormone,AMH)水平;比较两组IVF过程中促性腺激素(gonadotropin,Gn)应用天数、Gn用量、人绒毛膜促性腺激素(human chorionic gonadotrophin,HCG)扳机日单个成熟卵泡(直径≥14mm)的E2水平、获卵数、可移植胚胎数、优质胚胎数。结果两组Gn天数及Gn用量,差异无统计学意义(P0.05);观察组HCG日单个成熟卵泡E2水平、获卵数、可移植胚胎数较对照组高(P0.05);优胚数亦呈上升趋势,但差异无统计学意义(P0.05)。结论坤泰胶囊能提高IVF-ET中POR患者HCG日单个成熟卵泡E2水平,增加获卵数和可移植胚胎数,有助于改善IVF结局。
