联合检测血清、腹腔液及腹腔引流液CA125、CA199及IL-6对子宫内膜异位症的病情评估

【摘要】:目的:测定子宫内膜异位症(endometriosis,EMS)患者血清及腹腔液中CA125、CA199及白细胞介素-6(IL-6)的浓度,探讨三种因子的变化与EMS的关系,寻找诊断及检测疾病发展较为准确的方法。方法:选取了40例EMS患者(实验组),按r-AFS分期法分为I-IV期,其中EMSI、II期16例,III、IV期24例;40例子宫肌瘤患者(对照组)。采用免疫化学发光法检测两组患者血清及腹腔液中三种因子的浓度,比较两者的差别。结果:1、EMS患者血清CA125、CA199及IL-6的检测值、阳性率均显著增高于对照组;2、EMS患者腹腔液CA125、CA199及IL-6的检测值、阳性率均显著增高于对照组;3、EMSI、II期患者血清中三种因子检测与对照组无显著性差异(P0.05)。而腹腔液中三种因子水平显著高于对照组(P0.01);EMSIII、IV期患者血清与腹腔液三种因子均高于对照组(P0.05);EMSI、II期患者与III、IV期患者血清检测差异显著(P0.05);4、血清CA125、CA199及IL-6联合测定用于诊断EMS的敏感性及特异性分别为55%及100%;5、术前、术后六月血清CA125、CA199及IL-6比较差异有显著性,P0.01。结论:EMS患者血清和腹腔液中CA125、CA199及IL-6与EMS的发生、发展密切相关,三种因子血清学联合检测可提高子宫内膜异位症诊断的特异性;通过测定血清CA125、CA199及IL-6可作为其病情评估及判断其预后的指标。
[Abstract]:Objective: to determine the concentration of CA125,CA199 and interleukin-6 (IL-6) in serum and peritoneal fluid of patients with endometriosis (endometriosis,EMS), to explore the relationship between the changes of three factors and EMS, and to find a more accurate method for the diagnosis and detection of the development of endometriosis. Methods: forty patients with EMS (experimental group) were divided into I-IV stage according to r-AFS staging. Among them, there were 16 cases with EMSI,II stage and 24 cases with uterine leiomyoma (control group). The concentration of three factors in serum and celiac fluid of the two groups were detected by immunochemiluminescence method. Results the positive rates of CA125,CA199 and IL-6 in the patients with CA125,CA199 were significantly higher than those in the control group (P < 0.01), and the positive rates were significantly higher than those in the control group (P < 0.01). 3 there was no significant difference between the three factors in the serum of EMSI stage II patients and the control group (P0.05). The levels of three factors in the peritoneal fluid were significantly higher than those in the control group (P0.01) and the serum levels of the three factors in the patients with EMSIIIIV stage IV were significantly higher than those in the control group (P0.05) and the serum levels of the patients with EMSI stage II and III,IV were significantly higher than those in the control group (P0.05). 4. The sensitivity and specificity of serum CA125,CA199 and IL-6 in the diagnosis of EMS were 55% and 100%, respectively. There was a significant difference in serum CA125,CA199 and IL-6 between preoperative and postoperative 6 months (P 0.01). Conclusion the occurrence and development of CA125,CA199 and IL-6 in serum and peritoneal fluid are closely related to the development of EMS. The combined detection of three factors can improve the specificity of diagnosis of endometriosis. Serum CA125,CA199 and IL-6 can be used as an index to evaluate the disease and to judge the prognosis.
