盆腔器官脱垂患者阴道壁、骶主韧带中神经纤维特点及VIP、NPY表达特点和意义

【摘要】:目的:研究盆腔器官脱垂(pelvic organ prolapse,POP)患者阴道壁、骶主韧带的组织结构特点、神经纤维损伤及血管活性肠肽(VIP)、神经肽Y(NPY)表达的特点,分析与POP疾病发生的相关性。 方法:选择广西医科大学第一附属医院妇产科2013年1月~2014年3月收治的35例患者,分为三组:POP(Ⅰ-Ⅱ)组7例、POP(Ⅲ-Ⅳ)组8例和对照组(非POP患者)20例。标本选取阴道前壁、子宫骶韧带及主韧带组织,采用HE染色和免疫组化染色,光镜下观察各组切片组织结构及神经纤维束特点,计算神经纤维束密度,采用Leica Owin系统测定血管周围VIP、NPY灰度值,采用t检验、方差分析(AVOVA)检验方法对数据进行分析。 结果:⑴阴道分娩次数对照组与POP(Ⅰ-Ⅱ)组、POP(Ⅲ-Ⅳ)组分别比较,差异均有统计学意义(P<0.05),POP(Ⅰ-Ⅱ)组与POP(Ⅲ-Ⅳ)组比较,差异无统计学意义(P>0.05);⑵HE染色结果显示POP患者阴道壁、骶主韧带组织结构薄弱,其严重程度与POP病情呈正相关;骶韧带和主韧带组织结构具有相似性;⑶POP患者阴道壁、骶韧带及主韧带神经纤维损伤主要表现为神经纤维束结构改变,与POP病情呈正相关,而神经纤维束密度三组之间比较,差异均无统计学意义(P>0.05),同一患者骶、主韧带神经纤维束密度比较,差异无统计学意义(P>0.05);⑷阴道壁、骶韧带及主韧带组织中血管周围VIP、NPY灰度均值三组间比较,差异均有统计学意义(P<0.05),对照组<POP(Ⅰ-Ⅱ)组<POP(Ⅲ-Ⅳ)组,灰度均值越高表示VIP、NPY表达量越低。 结论:⑴阴道分娩次数是阴道壁、骶主韧带神经损伤及POP发生的主要高危因素。⑵骶韧带和主韧带组织结构相似,神经纤维束密度相当,提示骶、主韧带具有同源性。⑶POP患者阴道壁、骶主韧带组织中神经纤维损伤主要表现为神经纤维束结构的改变,而非神经纤维束密度,且VIP、NP表达量减少,导致盆底组织结构改变而发生POP,提示VIP、NPY表达在POP的发生、发展中起重要作用。
