高强度聚焦超声消融治疗子宫腺肌病的临床结果分析

【摘要】:研究背景: 子宫腺肌病是由子宫内膜组织异位至子宫肌层所致,是比较常见的妇科疾病,子宫内膜基底层损伤及局部高水平雌激素的刺激可能与本病发病相关。子宫腺肌病多发生于育龄期及绝经前经产妇,主要临床表现是进行性痛经、经量增多、经期延长、不孕等,明显影响了患者的生活质量。目前,子宫腺肌病的治疗方法主要有药物治疗和手术治疗。对于临床症状较轻、有生育要求及近绝经的患者,可使用药物治疗控制症状。对于症状明显、药物无效、没有生育要求的患者,子宫切除术是唯一可以根治子宫腺肌病的方法。为了减少手术并发症发生率,提高患者术后生活质量,保守性手术(子宫腺肌病病灶挖除术、子宫内膜去除术、子宫动脉栓塞术及盆腔神经切除术等)及超声消融治疗等多种治疗手段已经用于子宫腺肌病的临床治疗中。高强度聚焦超声(HIFU)消融是一种新型治疗技术,将体外低强度超声波聚焦于体内靶区域,在1秒内靶组织温度快速上升至60℃以上,造成靶组织发生不可逆凝固性坏死,却不损伤周围正常组织。该技术在子宫肌层良性病变治疗方面的应用为子宫腺肌病的治疗提供了新的治疗手段。 本研究回顾性分析接受HIFU消融治疗的子宫腺肌病患者,随访患者术后临床症状改善情况及并发症发生情况。通过比较分析前壁子宫腺肌病和后壁子宫腺肌病消融效率、治疗强度、能效因子及不良反应发生率等情况来判断HIFU消融治疗不同部位子宫腺肌病的疗效及可能发生的并发症等,,探索子宫腺肌病的位置对HIFU治疗效果的影响。 目的: 1.回顾性分析HIFU消融治疗子宫腺肌病的临床疗效。 2.通过分析比较HIFU消融治疗不同部位子宫腺肌病的消融效率、治疗强度、能效因子及不良反应发生率的情况等,探索子宫腺肌病的位置对HIFU治疗效果的影响。 资料和方法: 1.回顾性分析2011年09月至2012年06月在重庆医科大学附属第一医院HIFU中心经临床、MRI及超声检查确诊为子宫腺肌病并接受HIFU治疗的患者58例。随访HIFU治疗1个月、3个月、6个月、12个月患者的痛经、月经量、贫血改善情况及不良反应发生情况。对高强度聚焦超声消融治疗子宫腺肌病的临床应用价值进行探讨。 2.2013年01月至2013年10月在重庆医科大学附属第一医院HIFU中心确诊为子宫腺肌病并接受HIFU治疗的患者45例,根据腺肌病灶位置分为前壁组、后壁组,比较分析HIFU消融治疗不同部位子宫腺肌病的消融效率、治疗强度、能效因子及不良反应发生率的情况。 结果: 1.术后1、3、6、12个月随访患者的痛经疼痛评分及月经量评分与术前相比均降低并且具有统计学意义(P<0.05),患者的贫血情况也明显改善。 2.比较前壁组与后壁组超声消融的结果,两组间在辐照时间、治疗强度、总能量、非灌注区体积、消融率、能效因子(EEF)、单位体积所需辐照时间及副反应发生方面的差异均无统计学意义。 结论: 1.超声消融治疗子宫腺肌病临床疗效好、安全性好,副作用小,是适用于子宫腺肌病患者的一种非侵入性、安全、有效的新治疗方式。 2.子宫腺肌病灶的位置不是影响超声消融效果的主要因素,前、后壁子宫腺肌病均适合超声消融治疗。
