SCCE、Villin在宫颈腺癌中的表达及意义

【摘要】:目的:宫颈癌在女性恶性肿瘤中排名第二,基本分为腺癌和鳞癌,宫颈腺癌的发生率要明显低于宫颈鳞癌,据统计显示腺癌约占宫颈癌的5%-8%,但近年来,有研究表明,宫颈腺癌的发病率有所增加,由于宫颈腺癌,不容易被发现等原因,使宫颈腺癌的预后远远差于宫颈鳞状细胞癌,对于宫颈腺癌做出准确的病理诊断对于指导临床尤为重要,本实验主要是研究人角化层糜蛋白酶(stratumcorneum chymotryptic enzyme, SCCE),绒毛蛋白(Villin)在宫颈腺癌发展过程的各阶段中的表达情况,并分析它们对于诊断宫颈腺癌及判断预后的重要意义。 方法:采用免疫组织化学法检测宫颈腺癌45例、宫颈腺上皮内瘤变(cervicalglandular intraepithelial neoplasia, CGIN)24例和正常宫颈组织24例中SCCE,Villin的表达及45例宫颈腺癌中雌激素受体(estrogen receptor,ER)、癌胚抗原(carcino-embryonic antigen,CEA)、波形蛋白(Vimentin)的表达情况,采用Western blotting方法检测10例宫颈腺癌和5例正常宫颈组织中SCCE、Villin的表达情况,结合患者临床病理资料和出院后的随访情况,统计学分析SPSS16.0统计软件包进行。以P0.05为差异有统计学意义。 结果: 1免疫组织化学染色结果 1.1SCCE、Villin在宫颈腺癌、宫颈腺上皮内瘤变和正常宫颈中的表达 对免疫组化结果进行统计学分析:SCCE、 Villin蛋白在宫颈腺癌组、CGIN组中的表达与正常宫颈组的表达比较,进行统计学分析,差异具有显著性(P0.05),宫颈腺癌组与CGIN组间比较,差异无显著性(P>0.05)。 1.2SCCE、Villin在宫颈腺癌中的表达情况与宫颈腺癌中ER、CEA、Vimentin表达的相关性 对免疫组化结果进行统计学分析:SCCE表达与Villin呈正相关(r=0.434,P0.05)、与Vimentin呈负相关(r=㧟0.373,P0.05)、与ER呈负相关(r=㧟0.629,P0.05)、与CEA表达呈正相关(r=0.434,P0.05)(见表3);Villin表达与Vimentin呈负相关(r=㧟0.484,P0.05)、与ER呈负相关(r=㧟0.883,P0.05)、与CEA表达呈正相关(r=0.775,P0.05)(见表4)。 1.3SCCE、Villin的表达与宫颈腺癌临床病理参数之间的关系 SCCE、Villin在宫颈腺癌I期中表达率低于II+III+IV期中的表达率,差异有显著性,具有统计学意义(P0.05),在高分化宫颈腺癌中的表达率低于中、低分化宫颈腺癌中的表达率,差异有显著性,具有统计学意义(P0.05),在有淋巴结转移的宫颈腺癌中的表达率高于没有发生淋巴结转移患者的表达率,差异有显著性,具有统计学意义(P0.05)。 1.4宫颈腺癌预后的多因素分析 Cox比例风险模型多因素分析显示:临床分期、组织分级、淋巴结转移情况及SCCE的表达情况可以显著影响宫颈腺癌患者的生存时间(P0.05),但Villin的表达情况对宫颈腺癌患者生存时间的影响不大。 1.5SCCE、Villin蛋白的表达及临床病理参数与宫颈腺癌预后的单因素分析 临床分期I期组,组织分级高分化组,淋巴结未发生转移组,SCCE阴性表达组的生存率显著高于临床分期II+III+IV期组,组织分级中、低分化组,,淋巴结发生转移组,SCCE阳性表达组的生存率,并且差异有显著性,具有统计学意义(P0.05)。 2Western blot结果 SCCE、Villin在宫颈腺癌组中表达率显著高于正常宫颈组(P0.05),差异有显著性,具有统计学意义。 结论 1、SCCE、Villin在正常宫颈、CGIN和宫颈腺癌组中的表达率是呈上升趋势;在宫颈腺癌中,随着临床分期的增加、组织分级的降低及淋巴结转移的发生,SCCE、Villin的表达率都是逐渐升高的,说明SCCE及Villin的表达可能与宫颈腺癌的发生、发展及转移有关。 2、SCCE、Villin、ER、CEA及Vimentin联合检测可以提高宫颈腺癌的诊断准确率。 3、SCCE的表达可以作为判断宫颈腺癌患者预后的指标之一。
