胸腔积液对急性胰腺炎严重性及预后影响研究

【摘要】：目的：研究胸腔积液对急性胰腺炎病情严重性及预后的影响。方法：研究临床197例急性胰腺炎患者，回顾性统计分析其临床资料，探讨胸腔积液发生率及其与患者性别、年龄、48小时Ranson评分、住院时间、局部并发症（假性囊肿、坏死性胰腺炎、胰腺脓肿）以及全身并发症（休克、肺功能不全、急性肾功能衰竭）之间的关系。结果：197例AP患者，MAP158例（80.20%），SAP39例（19.80%）。胸腔积液患者总共27例（13.71%），158例MAP中存在胸腔积液患者10例（6.33%），，39例SAP中存在胸腔积液患者17例（43.59%），SAP组胸腔积液发生率与MAP组有明显差异（P=0.000）；胸腔积液与年龄、Ranson评分、全身并发症等指标有明显的相关性（P0.05），而与性别、局部并发症未见明显差异。结论：胸腔积液对于急性胰腺炎病情严重性有密切关系；AP患者入院应行常规胸部影像学检查；胸腔积液的AP患者需积极预防其他严重并发症的发生。
[Abstract]:Objective: to study the effect of pleural effusion on the severity and prognosis of acute pancreatitis. Methods: the clinical data of 197 patients with acute pancreatitis were analyzed retrospectively. The incidence of pleural effusion and its association with sex, age and 48 hours Ranson score, hospital stay, local complications (pseudocyst) were studied. Relationship between necrotizing pancreatitis, pancreatic abscess, and systemic complications (shock, pulmonary dysfunction, acute renal failure). Results the incidence of pleural effusion was significantly different between MAP group and MAP group. There were 27 patients with pleural effusion and 10 patients with pleural effusion in 10 patients with pleural effusion in 10 patients with pleural effusion in 39 patients with SAP and 17 patients with pleural effusion in SAP group with 43.59SAP group and MAP group in which the incidence of pleural effusion was significantly different from that in MAP group. Luminal effusion and age Ranson score, There was a significant correlation between systemic complications and other indexes (P 0.05), but there was no significant difference with sex and local complications. Conclusion: pleural effusion is closely related to the severity of acute pancreatitis.
