老年高血压急性脑出血患者炎症因子水平与周围脑组织水肿的相关性

【摘要】:目的探讨老年高血压急性脑出血患者不同时间点炎症因子水平变化及其与周围脑组织水肿的关系。方法选择83例老年高血压急性脑出血患者,分别在急性脑出血6 h、12 h、24 h、3 d、7 d内用酶联免疫法测定患者血清及血肿液中炎症因子的浓度并CT检查动态观察血肿及其周围低密度,分析炎症因子与周围脑组织水肿的关系。同时选取45例健康成年体检者作为正常对照组。结果白细胞介素(IL)-4、IL-6、IL-8浓度均在≥24且72 h达到峰值,第3~7天开始下降,但其水平均高于正常对照组(P0.05);而IL-10水平呈持续增高状态,7 d内未观察到峰值,除6 h内差异无统计学意义外,其余时间患者水平均高于正常对照组(P0.05)。脑出血量与脑水肿量呈正相关,均在≥12且24 h达峰值,且此时相关系数最高(r=0.698)。与正常对照组比较,高血压脑出血患者血清和血肿液中4种炎症因子水平均较高(P0.05);而患者血清和血肿液中4种炎症因子水平比较差异无统计学意义(P0.05)。IL-4、IL-6、IL-8与水肿量均呈正相关,而IL-10与水肿量呈负相关;四种炎症因子与出血量均无线性相关性(P0.05)。结论老年高血压脑出血患者血清IL-4、IL-6、IL-8、IL-10水平均增高,并随时间动态变化;四种炎症因子均相互影响,与周围脑组织水肿相关性由强到弱依次为IL-4、IL-10、IL-6、IL-8。
[Abstract]:Objective to investigate the changes of inflammatory factors at different time points in elderly patients with hypertensive acute intracerebral hemorrhage (AICH) and their relationship with edema of peripheral brain tissue. Methods Eighty-three elderly patients with hypertensive acute intracerebral hemorrhage were selected. The concentrations of inflammatory factors in serum and hematoma fluid were determined by enzyme-linked immunosorbent assay (Elisa) within 6 h, 12 h, 24 h and 3 d after acute intracerebral hemorrhage, and the dynamic observation of hematoma and its surrounding low density was performed by CT examination. To analyze the relationship between inflammatory factors and edema of peripheral brain tissue. At the same time, 45 healthy adults were selected as the normal control group. Results the concentration of IL-8 was more than 24 and reached the peak value at 72 h. It began to decrease on the 7th day, but the level was higher than that of the normal control group (P0.05), but the level of IL-10 was not observed within 7 days after the increase of IL-10. Except in 6 hours, the level of patients was higher than that of normal control group (P0.05).
