ICU病原菌分布特点与耐药性研究

【摘要】:目的探讨重症监护病房(ICU)病原菌的分布及耐药性研究,为控制ICU的感染提供参考依据。方法选取2014年1月-2015年6月医院ICU住院患者送检的712份标本进行细菌培养,采用BD公司Poenix 100全自动细菌鉴定/药敏系统对病原菌进行鉴定以及药敏试验,数据采用SPSS 19.0软件进行统计分析。结果 712份标本中有386份阳性,阳性率为54.21%;共检出480株病原菌,以革兰阴性菌为主,共354株,占73.75%,其次为真菌67株,占13.96%,革兰阳性菌59株,占12.29%;鲍氏不动杆菌对多黏菌素耐药率最低为0,肠球菌属对利奈唑胺、替考拉宁和万古霉素耐药率最低,均为0,检出的所有真菌对两性霉素B耐药率均为0。结论 ICU住院患者中非发酵菌和真菌的检出率高,且多种病原菌耐药性较其他科室严重,需加强ICU病原菌及耐药性的监测,根据药敏的结果合理、正确的使用抗菌药物,减少耐药菌感染的发生。
[Abstract]:Objective to study the distribution and drug resistance of pathogenic bacteria in intensive care unit (ICU). Methods from January 2014 to June 2015, 712 samples of hospitalized patients in ICU were selected for bacterial culture. Poenix 100 automatic bacterial identification / drug sensitivity system of BD company was used to identify pathogenic bacteria and drug sensitivity test. The data were analyzed by SPSS 19.0 software. Results 386 of 712 specimens were positive, the positive rate was 54.21, 480 pathogenic bacteria were detected, 354 were Gram-negative bacteria (73.75cm), 67 were fungi (13.96), 59 were Gram-positive bacteria. The resistance rate of Acinetobacter baumannii to polymyxin was the lowest, and that of Enterococcus to linazolamide, teicoplanin and vancomycin was the lowest, all the fungi were found to be resistant to amphotericin B. Conclusion the detection rate of non-fermentative bacteria and fungi in ICU patients is high, and the drug resistance of many pathogens is more serious than that of other departments. It is necessary to strengthen the surveillance of pathogens and drug resistance in ICU. According to the results of drug sensitivity, the antibiotics should be used correctly. Reduce the incidence of drug-resistant bacteria infection.
