急性心肌梗死3h内瑞替普酶溶栓与PCI治疗对左心功能影响的对比分析

【摘要】：目的对比急性心肌梗死3 h内应用瑞替普酶溶栓治疗与急诊PCI治疗对梗死后心室重构及左心功能的影响。方法 98例患者分为瑞替普酶溶栓治疗组与急诊PCI治疗组,对两组患者心功能相关指标如NTPro BNP水平、收缩期末容量指数、舒张期末容量指数、射血分数、室壁运动积分指数进行比较。结果急性心肌梗死3 h内进行溶栓治疗与PCI治疗对氨基末端脑钠肽前体(NT-Pro BNP)水平、收缩期末容量指数、舒张期末容量指数、射血分数、室壁运动积分指数比较均无统计学差异(P0.05)。结论急性心肌梗死3 h内溶栓治疗与PCI治疗对心功能影响程度相当,均为有效的再灌注治疗方法,降低并延缓心室重构发生,降低心力衰竭的发生率。对那些没有条件开展直接PCI治疗地区的急性ST段抬高型心肌梗死患者,进行溶栓并在适当时间内行PCI治疗,是一种可行的治疗策略。
[Abstract]:Objective to compare the effects of reteprase thrombolytic therapy and emergency PCI therapy on ventricular remodeling and left ventricular function in patients with acute myocardial infarction (AMI) within 3 hours. Methods 98 patients were divided into two groups: Retipase thrombolytic therapy group and emergency PCI treatment group. The cardiac function related indexes such as NTPro BNP level, end-systolic volume index, end-diastolic volume index, ejection fraction were measured in two groups. The wall motion integral index was compared. Results there was no significant difference between thrombolytic therapy and PCI therapy in the levels of NT-Pro BNP, end-systolic volume index, end-diastolic volume index, ejection fraction and wall motion integral index of acute myocardial infarction (P0.05). Conclusion Thrombolytic therapy within 3 hours after acute myocardial infarction has the same effect on cardiac function as PCI treatment, which is an effective method for reperfusion therapy, which can reduce and delay the occurrence of ventricular remodeling and reduce the incidence of heart failure. Thrombolysis and PCI therapy in patients with acute ST-segment elevation myocardial infarction without the conditions for direct PCI treatment is a feasible treatment strategy.
