多处脑挫裂伤患者的救治及预后分析

【摘要】:目的回顾性分析研究大脑实质多处挫裂伤患者的临床表现、影像学检查以及预后恢复情况,探讨多处脑挫裂伤患者的临床救治的量化指标,从而为临床医生的工作提供参考。方法对150例大脑挫裂伤总体积在30ml以上的多处脑挫裂伤患者的临床救治及预后进行回顾性分析,根据多处脑挫裂伤总体积50ml和≥50ml以及GCS评分≤8分和8分进行分组研究,通过分析患者GOS预后评分,比较各组患者的预后情况。结果(1)对于多处脑挫裂伤总体积在30ml-50ml的A组病人,手术或非手术处理后患者的残、死率无统计学差异(P0.05);对于脑挫裂伤总体积≥50ml的B组患者,手术与非手术治疗的残、死率有显著统计学差异(P0.05),与非手术者比较,B组的手术者GOS 4-5分百分率较A组升高了31.9%,GOS 1-3分百分率较A组下降了31.9%。(2)对于多处脑挫裂伤GCS8分的C组病人,手术或是非手术处理后患者的残、死率无统计学差异(P0.05);但对于GCS≤8分的D组病人,手术组和非手术组预后比较,结果显示致残、致死率具有显著性差异(P0.05)。D组的手术者GOS4-5分百分率较C组升高24.7%,GOS 1-3分百分率较C组降低了下降了24.7%。结论对于多处脑挫裂伤的患者,若大脑部位多处挫裂伤总体积之和50ml以及GCS8分则多考虑非手术治疗;若大脑部位多处挫裂伤总体积之和≥50ml以及GCS评分≤8分时应考虑积极手术治疗;若以脑挫裂伤总体积和GCS评分判断的患者均指向手术治疗,则尽可能行开颅去骨瓣手术治疗,若一项指标指向手术治疗,则可根据患者的病情变化、年龄、体质情况等多种因素综合考虑是否行手术治疗。这样能够尽早的决策手术与否,从而为手术患者节约观察时间,最大程度的减轻脑挫裂伤患者的后遗症,减轻患者的住院时间和经济负担。
