双相情感Ⅰ型和Ⅱ型障碍中五因子人格特质与其情绪状态的关联

[背景]双相情感障碍患者通常有高冲动性和高感觉寻求特质倾向,这两个特质可能对双相情感Ⅰ型和Ⅱ型障碍患者的情绪状态造成不同的影响。[方法]23名双相情感Ⅰ型障碍患者、22名双相情感Ⅱ型障碍患者和64名健康志愿者完成五因子模型Zuckerman-Kuhlman人格问卷(ZKPQ)以测量人格特质,完成Plutchik-van Praag抑郁量表(PVP)、Mood Disorder Questionnaire (MDQ)和Hypomania Checklist-32 (HCL-32)以评估情绪状态。以方差分析评估各组在各量表上的得分状况,并通过多元线性回归模型以ZKPQ得分为自变量预测各组PVP, MDQ和HCL-32得分。[结果]双相情感Ⅰ型和Ⅱ型障碍患者在ZKPQ冲动感觉寻求(在冲动性亚尺度也有同样表现)、神经质-焦虑、攻击-敌意性分量表以及PVP, HCL-32上的得分均比健康志愿者的分数高。双相情感Ⅰ型障碍患者在MDQ, ZKPQ一般感觉寻求亚尺度上的得分比健康志愿者高,且在ZKPQ冲动感觉寻求分量表(以及一般感觉寻求亚尺度)和MDQ上的得分均高于双相情感Ⅱ型障碍患者。在双相情感Ⅰ型障碍患者中,ZKPQ一般感觉寻求得分正性预测MDQ得分,ZKPQ活跃性得分正性预测HCL-32得分。在双相情感Ⅱ型障碍患者中,ZKPQ攻击-敌意性正性预测HCL-32得分。而在健康志愿者中,ZKPQ一般感觉寻求正性预测MDQ和HCL-32得分,而该分量表与ZKPQ神经质-焦虑一同正性预测PVP得分。[结论]我们的结果提示冲动感觉寻求人格特质和一般感觉寻求亚尺度可以帮助刻画双相情感Ⅰ型和Ⅱ型障碍。
