【摘要】:[目的]埋伏牙是临床上较常见的一种错(?)畸形,对患者口腔正常功能的发挥和美观有着较大影响,可引起牙弓完整性的破坏、咬合关系紊乱、邻牙牙根吸收等。牙弓中的任何牙都可能发生埋伏,由于种族、人种以及调查对象不同,埋伏牙的患病情况统计结果并不相同。对埋伏牙的精确定位是制定治疗方案和选择手术路径的依据。锥形束CT三维重建可清楚显示埋伏牙的位置及与周围组织的结构关系,本研究利用CBCT对埋伏牙患病情况资料进行全面收集整理,充分利用CBCT的优势,对埋伏中切牙的位置以及牙根的冠根成角情况进行统计分析,并对其影响疗程长短的因素作出分析,以便能更好地为临床应用提供理论基础和科学指导。[材料与方法]第一部分:在我院正畸科2012年10月份至2014年10月份就诊的6348例患者中,以确诊的574例埋伏牙患者为研究对象,对其性别、年龄、部位和数量等基本情况进行调查研究。第二部分:以193例患者,212颗上颌埋伏中切牙为研究对象,将其CBCT影像转化为DICOM格式,对其位置进行测量和冠根成角的统计分析。第三部分:以25例上颌埋伏中切牙成功导萌的患者为研究对象。分析导萌疗程,上颌埋中切牙切缘至(?)平面的距离(d),上颌埋伏中切牙倾斜角度(α),上颌埋伏中切牙的垂直高度,上颌埋伏中切牙冠根成角度数(β)及年龄(Y)。[结果l埋伏牙在口腔正畸患者中的患病率为9.04%,以上颌好发,上颌尖牙患病率最高,且女性多于男性。上颌埋伏中切牙44.04%为倒置埋伏阻生,50.47%为唇侧埋伏,16.98%为腭侧埋伏,29.24%为近中埋伏,26.89%为远中埋伏,切缘至(?)平面的平均距离为7.5mm。上颌埋伏中切牙引起邻牙牙根吸收发生率为12.60%。上颌埋伏中切牙的导萌疗程与患者年龄及d成正相关;冠根成角度数越小,畸形程度越严重,越难牵引;与a成负相关。[结论]在正畸门诊患者中,埋伏牙的患病率较高,尤其是上颌尖牙,提示正畸医生在患者就诊时要注意埋伏牙的早期诊断及治疗。上颌埋伏中切牙位置变化较大,倒置埋伏发生率较高,造成邻牙牙根吸收的发生率也较高,应引起正畸医生的重视。上颌埋伏中切牙的位置和患者的年龄可为预测导萌疗程提供重要依据。
[Abstract]:[Objective] the embedded tooth is a common malformation in clinic, which has great influence on the normal function and aesthetic appearance of the oral cavity, which can cause the integrity of the dental arch, the disorder of occlusal relations, and the absorption of the adjacent teeth. Any tooth in the dental arch may be ambushed. The prevalence of ambushed teeth is not the same because of race, ethnicity, and different subjects. The accurate location of the ambush is the basis for formulating the treatment plan and selecting the surgical path. Cone beam CT reconstruction can clearly show the location and the relationship with the surrounding tissue structure of impacted teeth, this study uses CBCT of impacted teeth prevalence data were collected comprehensively, make full use of the advantages of CBCT, statistical analysis of the incisor in the ambush position and root crown angulation situation and its influence on the the duration of factors analysis, in order to provide theoretical basis and scientific guidance for clinical application. [materials and methods] the first part: in our hospital orthodontic department from October 2012 to October 2014, 574 cases of patients with impacted teeth were selected as the research objects, and their gender, age, location and quantity were investigated. The first part was 6348 cases. The second part: Taking 193 patients and 212 impacted maxillary central incisors as subjects, the CBCT images were transformed into DICOM format, and the location of Wacom was analyzed by statistical analysis. The third part: with 25 cases of maxillary impacted incisors, the patients were successfully guided. The distance from the incisor to the plane of the maxillary central incisor, the inclination angle of the maxillary central incisor, the vertical height of the maxillary central incisor, the angle of the central incisor and the age (Y) of the maxillary central incisor were analyzed. [results the prevalence rate of L ambushed teeth in orthodontic patients was 9.04%, upper jaw and maxillary canines were the highest, and women were more than men. 44.04% of the maxillary embedded central incisors were inverted impacted, 50.47% were labial ambush, 16.98% were palatal ambush, 29.24% were proximal mesopore, 26.89% were distal distal, and the average distance from the incisal edge to the plane was 7.5mm. The incidence of root absorption of adjacent teeth caused by incisor in the maxillary ambush was 12.60%. The course of guided eruption of the maxillary embedded central incisor is positively related to the age of patients and D. The smaller the crown angle, the more serious the degree of deformity is, the more difficult it is to draw. It has a negative correlation with a. [Conclusion] in orthodontic outpatients, the prevalence of impacted teeth is high, especially for maxillary canines. It is suggested that orthodontic doctors should pay attention to early diagnosis and treatment of dental impacted teeth when visiting patients. The position of incisor changes in the maxillary ambush, and the incidence of inverted ambush is high, and the incidence of root absorption is high, which should be paid attention by the orthodontic doctors. The position of the incisors in the maxillary ambush and the age of the patients can provide an important basis for predicting the course of guidance.
