补肾固齿丸辅助牙周基础治疗的牙槽骨改变的研究

【摘要】：目的：使用补肾固齿丸辅助牙周基础治疗，应用锥形束CT(Cone Beam ComputedTomography,CBCT)及相关软件检测牙周炎患者牙槽骨的改变，以观察补肾固齿丸辅助牙周基础治疗的临床疗效。方法：本次研究采用对照实验法，符合中医辩证的阴虚火旺型诊断标准的慢性牙周炎患者40例，分为给药组（n=20）和对照组（n=20）。治疗前检查并记录各项临床指数，包括临床附着丧失（clinicalat,CAL）、牙周袋探诊深度（crobing depth,PD）、牙龈探诊出血指数（bleeding on probing,BOP）、出血指数（bleeding Index,BI）和中医症候评分，治疗前通过CBCT对患者进行影像拍摄，再由两名口腔医师分别在图像上重复测量牙槽骨高度。所有患者均接受牙周基础治疗，同时给药组服补肾固齿丸辅助治疗、对照组服维生素C，3个月后行二次临床检查及CBCT检查，统计对比两组患者治疗前、治疗后的临床指标及CBCT测量的牙槽骨高度。将治疗前后牙槽骨高度变化值、中医症候评分变化值作为两个变量，进行线性相关分析。结果：1、两名口腔医师的测量结果不存在统计学差异；2、CBCT测量结果显示两组患者治疗后平均牙槽骨高度均较治疗前增加，且给药组的牙槽骨高度增加较对照组明显（P0.05），，两组患者治疗后部分牙周指数较治疗前增加，且给药组改善的程度明显优于对照组（P0.05）；3、直线相关分析结果显示，牙周炎患者的中医症候评分减少值与牙槽骨高度增加值呈正相关（r=0.945）。结论：1、补肾固齿丸作为牙周基础治疗的辅助手段有助于进一步促进牙周炎患者的牙槽骨修复。2、锥形束CT及相关软件可作为临床科研中的监测手段，通过准确测量牙周炎患者牙槽骨高度在临床治疗中的变化，能够较准确评估治疗方案的效果，同时可为进一步治疗提供指导意义。
[Abstract]:Objective: to apply the conical bundle CT(Cone Beam ComputedTomography to the basic periodontal treatment with Bushen Guya Pill. The changes of alveolar bone in patients with periodontitis were detected by CBCTand related software. Methods: 40 patients with chronic periodontitis who met the diagnostic criteria of yin deficiency and fire hypertrophy were studied by control experiment. The patients were divided into two groups: the control group (n = 20) and the control group (n = 20). The clinical indexes were examined and recorded before treatment, including clinical attachment loss. The depth of periodontal pouch exploration was used to detect PD, and the bleeding index of gingival probing was bleeds on probing BOP). Bleeding index index (BI) and TCM symptom score were taken by CBCT before treatment. The height of alveolar bone was measured repeatedly by two stomatologists respectively on the image. All the patients were treated with periodontal basic therapy, while the patients in the medication group were treated with Bushen Guya pills and the control group were treated with vitamin C. After 3 months, two clinical examinations and CBCT examination were performed. The clinical indexes and alveolar bone height measured by CBCT before and after treatment were compared statistically. The changes of alveolar bone height before and after treatment were compared. As two variables, the change value of TCM symptom score was analyzed by linear correlation. Results there was no statistical difference in the measurement results between the two dentists. 2 the results of CBCT measurement showed that the mean alveolar bone height in the two groups was higher than that before treatment, and the alveolar bone height in the administration group was significantly higher than that in the control group (P 0.05). The periodontal index of the two groups after treatment was higher than that before treatment, and the degree of improvement in the treatment group was significantly better than that in the control group (P 0.05). 3. The results of linear correlation analysis showed that there was a positive correlation between the decrease of TCM symptom score and the increase of alveolar bone height in patients with periodontitis. Conclusion Bushen Goodtooth Pill as an auxiliary method of periodontal basic treatment is helpful to further promote alveolar bone repair in periodontitis patients. Cone-beam CT and related software can be used as monitoring methods in clinical scientific research. The changes of alveolar bone height in patients with periodontitis can be accurately evaluated by clinical treatment and can provide guidance for further treatment.
