面部软组织侧貌美学指标的临床应用研究

【摘要】:背景:面部软组织侧貌美学指标能评价正畸正颌联合治疗患者术前术后的软组织变化,可以指导手术方案设计,尤其适合外科优先的正畸正颌联合治疗,也可用于评估患者术后的治疗效果,在正颌外科学中的应用十分广泛,是一种重要的研究手段和分析方法。目前,国内外有多种面部软组织侧貌美学指标体系,但是不同医师采用的指标体系不尽相同,种类繁多。因此筛选适合正颌外科的面部软组织美学指标,具有一定的临床意义。 目的:筛选敏感的面部软组织侧貌美学指标来测量正畸正颌联合治疗患者术前术后的软组织变化,评估面部软组织侧貌美学指标应用于正颌外科术前设计和术后评价的可行性。 材料方法:随机选取2007年1月至2013年7月来浙江大学医学院附属口腔医院就诊的14位正畸正颌联合治疗患者为研究对象。患者术前、术后拍摄侧面照及头颅侧位片,查阅相关文献选取合适的面部软组织侧貌美学指标并由同一名研究生三次测量取平均值:面型角、鼻唇角、零子午线(0线)至Sn点的距离(0-Sn值)、0线至Pos点的距离(0-Pos值)、面中及面下1/3的比例、口裂至颏下点与鼻下点至口裂的比例、上下唇突度、颏唇角、颏唇沟深度;将14名患者术前术后侧面照通过Photoshop CS8.0阴影化,由30名口腔医学生采用7级里克特量表进行容貌评价,取平均值作为对侧面貌美观度的评价;采用SPSS19.0统计软件包对结果进行统计分析。 结果:通过多重线性回归分析筛选出4项面部软组织侧貌的敏感指标:口裂至颏下点与鼻下点至口裂的比例(p=0.0010.01)、0-Pos值(p=0.0120.01)、0-Sn值(p=0.0130.01)、面型角(p=0.0130.01)。 结论:口裂至颏下点与鼻下点至口裂的比例、0-Pos值、0-Sn值、面型角这四项美学指标可以作为评价正颌术前及术后面部软组织侧貌的敏感指标。
[Abstract]:Background: the aesthetic index of facial soft tissue profile can evaluate the changes of soft tissue before and after orthodontic orthognathic treatment, and can guide the design of surgical scheme, especially suitable for orthodontic orthognathic combined therapy. It is widely used in orthognathic surgery and is an important method of research and analysis. At present, there are many kinds of aesthetic index system of facial soft tissue profile at home and abroad. However, the index system adopted by different doctors is different and there are many kinds. Therefore, it is of clinical significance to screen the aesthetic indexes of facial soft tissue suitable for orthognathic surgery. Objective: to screen sensitive aesthetic indexes of facial soft tissue profile to measure the changes of soft tissue before and after orthodontic orthognathic treatment. To evaluate the feasibility of preoperative design and postoperative evaluation of facial soft tissue profile in orthognathic surgery. Materials: from January 2007 to July 2013, 14 patients with orthodontic orthognathic combined treatment were randomly selected from the affiliated Stomatology Hospital of Zhejiang University Medical College. Before and after operation, lateral radiography and lateral cephalic radiography were taken. According to the relevant literature, the appropriate aesthetic indexes of facial soft tissue profile were selected and the average values were measured three times by the same graduate student: facial angle, nasolabial angle, The distance from zero meridian (0) to Sn (0) from 0 to Pos (0), the ratio of 1/3 between midplane and subfacial, the ratio of mouth to submental and subnasal to oral fissure, the degree of upper and lower lip protrusion, the angle of mental lip, and the depth of mental sulcus; 14 patients were shaded by Photoshop CS8.0 before and after operation, and 30 Stomatological students were evaluated with Richter scale of grade 7, and the average value was taken as the evaluation of contralateral beauty. The results were analyzed by SPSS19.0 software package. Results: by multiple linear regression analysis, four sensitive indexes of facial soft tissue profile were selected: the ratio of oral fissure to submental point and subnasal point to oral fissure: p0. 0010. 01 and 0. 0120. 01 ~ 0. 01 and p0. 013. 01, p0. 013. 01, p0. 013. 01, p0. 013. 01, p0. 013. 01, p0. 013. 01, p0. 013. 01, p0. 013. 01, respectively. Conclusion: the ratio of oral fissure to submental point to subnasal point to oral fissure is 0-Pos value 0-Sn value and facial angle can be used as a sensitive index to evaluate the facial soft tissue profile before and after orthognathic surgery.
