紫外线光能化对大颗粒喷砂酸蚀表面种植体稳定性的临床研究

【摘要】:目的:对紫外线光能化影响下的SLA(Sandblasted,Large-grit and acid-etched,大颗粒喷砂酸蚀)表面种植体骨结合能力进行研究,使用共振频率分析(Resonance Frequency Analysis,RFA)法分析紫外线光能化SLA表面种植体骨结合稳定系数(Implant Stability Quotients,ISQ)的变化趋势。方法:将32名在山西医科大学口腔医院种植科就诊的下颌后牙区个别牙缺失患者选为本文研究样本(就诊时间:2016年6-12月),共50颗SLA表面种植体,后牙区被植入24颗经紫外线照射的SLA表面种植体(实验组)和26颗未经紫外线照射的SLA表面种植体(对照组),将术后即刻、2周,4周、6周、8周、12周,六个时间点的种植体的稳定性采用OsstellTMISQ共振频率分析仪进行对比分析,并对各种植体近中向ISQ值、远中向ISQ值、颊侧ISQ值及舌侧ISQ值进行连续监测,取算数平均值,在SPSS18.0软件基础上采用统计学分析策略来进行分析对比两组数据的差异。结果:50颗种植体在植入时都取得了良好的初期稳定性,且在术后12周,种植体的留存率全部达到百分之百。实验组种植术后即刻,ISQ平均值为68.25±3.09,并在术后第2周下降至最低,随后出现逐渐上升。对照组种植术后即刻,ISQ平均值为68.88±3.66,术后呈现下降趋势且在术后第4周下降至谷底,随后出现逐渐上升。对比两组ISQ值,术后即刻差异无统计学意义(p0.05),而在术后的第2周,4周、6周、8周、12周统计学差异显著有统计学意义(P0.05)。结论:紫外线光能化SLA表面种植体可恢复种植体生物活性、加快种植体-骨结合速度、缩短愈合时间,体现出更高的临床稳定性,具有可靠的临床指导意义。
[Abstract]:Objective: to study the bone-binding ability of large scale grit and acid-etched surface implants under the influence of ultraviolet light (UV). Resonance frequency analysis (RFA) method was used to analyze the change trend of Implant Stability Quotients-ISQ (Implant Stability Quotients-ISQs) on the surface implants of ultraviolet photovoltaic SLA. Methods: 32 patients with mandibular posterior dentition from the Department of Implant, Department of Stomatology, Shanxi Medical University were selected. Individual dental defect patients were selected as samples in this study (visit time: June-December 2016, 50 SLA surface implants). Twenty-four SLA surface implants (experimental group) and 26 SLA surface implants without ultraviolet irradiation were implanted in the posterior tooth region (control group). The stability of implants at six time points was analyzed by OsstellTMISQ resonance frequency analyzer. The values of ISQ, ISQ, ISQ and ISQ were continuously monitored. Based on SPSS18.0 software, statistical analysis strategy was used to analyze and compare the differences between the two groups of data. Results all of the 50 implants achieved good initial stability at the time of implantation and 12 weeks after operation. The retention rate of implants was 100%. The mean ISQ of the experimental group was 68.25 卤3.09 immediately after implantation, and decreased to the lowest at the second week after implantation. In the control group, the mean value of ISQ was 68.88 卤3.66 immediately after implantation, which showed a downward trend and decreased to the trough at the 4th week after implantation, and then increased gradually. The ISQ values of the two groups were compared. There was no significant difference between the two groups immediately after operation (P 0.05), but there was significant difference in the second week, the second week, the 6th week, the 8th week and the 12th week after the operation. Conclusion: the surface implant of UV photoenergy SLA can recover the bioactivity of the implant. Accelerating the speed of implant-bone bonding, shortening the healing time, reflecting higher clinical stability, has reliable clinical significance.
