计算机导航辅助颅颌面骨纤轮廓修整的效果评价

【摘要】:目的:探讨计算机辅助导航系统在颅颌面部骨纤维异常增殖症轮廓修整中的应用效果。方法:回顾性收集2010年1月—2013年12月于上海交通大学医学院附属第九人民医院口腔颅颌面科就诊的33例颅颌面骨纤维异常增殖症患者,按照治疗方法分为两组:实验组17例,术前利用计算机导航系统的软件设计治疗方案,术中在该导航系统辅助下进行病变部分切除及轮廓修整术;对照组16例,用传统方法进行病变部分切除及轮廓修整术。术后1周和3个月拍摄颅颌面部CT并进行三维重建,应用三维检测软件Geomagic Qualify对手术区域与相应术前模拟设计区域的偏差、两组病例术后颅颌骨的对称性进行量化评价,并对出现误差的原因进行分析。结果:以镜像模型为参考模型,导航组中手术区域的平均偏差值为2.1285±1.2957 mm,非导航组中手术区域的平均偏差值4.4814±2.1940mm,导航组和非导航组手术区域的平均偏差值存在显著统计学差异(p0.01)。术后模型与术前模拟设计相比的平均偏差为2.1400±1.2482mm。结论:计算机导航系统辅助下进行的颅颌面部骨纤维异常增殖症轮廓修整术具有较高的精度,与传统手术方法相比能更好地恢复患者颅颌骨的对称性。
[Abstract]:Objective: to investigate the effect of computer aided navigation system in the contour dressing of craniomaxillofacial fibrous dysplasia. Methods: 9th patients affiliated to Shanghai Jiaotong University Medical College from January 2010 to December 2013 were collected retrospectively. 33 patients with craniomaxillofacial fibrous dysplasia, According to the treatment method, 17 cases in the experimental group were divided into two groups: 17 cases in the experimental group, 16 cases in the control group were treated with the software of computer navigation system before operation, partial resection and contours were performed during the operation assisted by the navigation system, and 16 cases in the control group were treated with computer navigation system. The traditional method of partial resection and contour dressing was used. Ct and 3D reconstruction of craniomaxillofacial region were taken 1 week and 3 months after operation. The deviation between the operation area and the corresponding preoperative simulated design area was analyzed by using Geomagic Qualify. The symmetry of craniomaxillary bone was quantitatively evaluated in both groups, and the causes of errors were analyzed. Results: the mirror image model was used as reference model. The mean deviation of operation area in navigation group was 2.1285 卤1.2957 mm, and that in non-navigation group was 4.4814 卤2.194 mm. There was significant difference in mean deviation between navigation group and non-navigation group. The average deviation of the design is 2.1400 卤1.2482 m.Conclusion: the contours of craniomaxillofacial fibrous dysplasia with the aid of computer navigation system have high accuracy. Compared with the traditional surgical methods, it can recover the symmetry of craniomaxillary bone.
