TWSG1在颅颌面骨纤维异常增殖症中的表达及意义

【摘要】:研究背景: 骨纤维异常增殖症又称骨纤维结构不良,是一种先天非遗传性骨组织疾病,男女均可患病,通常在儿童时期起病,青少年后才出现症状,病程较长,好发于颌面部,,颌面部FD临床表现为慢性无痛性肿胀隆起,进一步生长时引起面部左右不对称,侵犯临近组织时可表现如鼻塞、脓涕,咬合错位,眼移位伴复视,听力下降,头痛等症状。多数患者需要通过手术切除病损,往往造成外观畸形和功能障碍,给患者带来极大痛苦,并严重影响患者的生活质量,且局部切除术后复发概率高,给患者造成了极大的痛苦。研究与骨纤维异常增殖症发生和发展的相关因子,为骨纤维异常增殖症的非手术治疗寻找新的方法,降低治疗后的复发率具有重要的意义。 目的: 通过检测twisted gastrulation(TWSG1)蛋白在颅颌面骨纤维异常增殖症中的表达情况,分析其表达与颅颌面骨纤维异常增殖症患者的表达水平与年龄因素及预后关系,旨在研究TWSG1的表达在颅颌面骨纤维异常增殖症中的作用,可以进一步加强对TWSG1基因的认识,且可利用TWSG1进行颅颌面骨纤维异常增殖症的分子及基因治疗提供相关理论依据。 方法: 取44例颅颌面骨纤维异常增殖症石蜡标本及10例颌骨骨折外伤骨碎片标本,采用免疫组织化学Envision法检测TWSG1蛋白在颌面骨纤维异常增殖症和正常颌骨组织中的表达水平,将检测结果与患者的临床特征进行相关性分析。所有资料均运用SPSS19.0进行统计学分析,采用秩和检验,P0.05为差异有显著性意义。 结果: 1、颅颌面部骨纤维异常增殖症中TWSG1的阳性表达(44/44)在细胞质/膜内呈棕黄色颗粒如图所示,主要见于骨母细胞及成纤维细胞。而正常颌骨组织中TWSG1的表达阳性(2/10)。 2、FD组织TWSG1表达与年龄及预后关系FD组织TWSG1表达与年龄及预后关系见表1。FD患者TWSG1皆阳性表达,明显高于正常颌骨阴性对照组(P<0.05);未成年FD患者TWSG1表达也明显高于成年组(P<0.05);以预后复发与否分类,亦可见预后良好的FD患者TWSG1表达明显低于预后差的患者(P<0.05)。 结论: 1、TWSG1蛋白在颅颌骨骨纤维异常增殖症组织中表达上升并高表达,TWSG1可能促进了颌面部骨化性纤维瘤的发生与发展,并可能在其中发挥了重要作用。 2、FD的TWSG1的阳性表达水平与疾病发展进程密切相关,是FD患者预后不良的重要生物学指标。
[Abstract]:Background:. Dysplasia of bone fiber, also known as dysplasia of bone fiber, is a congenital non-hereditary bone tissue disease. Both men and women are ill. It usually starts in childhood, and symptoms appear only after adolescence. The course of disease is relatively long, and it is prone to occur in the maxillofacial region. The clinical manifestations of FD in maxillofacial region were chronic painless swelling and protuberance, which caused asymmetry of left and right face during further growth, and may be shown as nasal obstruction, purulent snot, malocclusion, eye displacement with diplopia and hearing loss when invading adjacent tissues. Headache and other symptoms. Most patients need surgery to remove the lesions, often resulting in appearance deformities and dysfunction, bring great pain to patients, and seriously affect the quality of life of patients, and local resection after the recurrence rate is high, It is of great significance to study the factors related to the occurrence and development of osteofibrillary dysplasia, to find a new method for non-operative treatment of osteofibrillary dysplasia, and to reduce the recurrence rate after treatment. Objective:. The expression of twisted gastrulation TWSG1 protein in craniomaxillofacial fibrous dysplasia was detected, and the relationship between the expression level and age factors and prognosis of patients with craniomaxillofacial fibrous dysplasia was analyzed. The purpose of this study was to study the role of TWSG1 expression in craniomaxillofacial fibrous dysplasia, and to further enhance the understanding of TWSG1 gene, and to use TWSG1 for molecular and gene therapy of craniomaxillofacial fibrous dysplasia. Methods:. Paraffin wax specimens from 44 cases of craniomaxillofacial fibrous dysplasia and 10 cases of traumatic bone fragments from maxillofacial fractures were used to detect the expression of TWSG1 protein in maxillofacial fibrous dysplasia and normal maxillofacial tissues by immunohistochemical Envision method. All the data were statistically analyzed by SPSS19.0, and the difference was significant by rank sum test (P0.05). Results:. 1. The positive expression of TWSG1 in craniomaxillofacial fibrous dysplasia (44 / 44) was brown in cytoplasm / membrane, mainly in osteoblasts and fibroblasts, while in normal maxillofacial tissues, the positive expression of TWSG1 was 2 / 10 / 10. 2the expression of TWSG1 and age and prognosis in FD tissue; the expression of TWSG1 in FD tissue and the relationship between age and prognosis were all positive in 1.FD patients. The expression of TWSG1 in the juvenile patients with FD was significantly higher than that in the adult group (P < 0.05), and the expression of TWSG1 in the patients with good prognosis was significantly lower than that in the patients with poor prognosis (P < 0.05). Conclusion:. 1the expression of TWSG1 protein in craniomaxillary fibrous dysplasia tissues increased and the high expression of TWSG1 may promote the occurrence and development of ossifying fibroma of maxillofacial region, and may play an important role in it. 2the positive expression level of TWSG1 in FD is closely related to the progression of the disease and is an important biological indicator of poor prognosis in FD patients.
