原子力显微镜下自锁托槽与国产直丝弓托槽滑过弓丝前、后弓丝表面结构的变化

【摘要】:目的:探讨自锁托槽与普通直丝弓托槽在牙齿移动过程中对0.018″×0.025″方形不锈钢弓丝表面结构产生的影响。 方法:对戴用Damon-Q自锁托槽和Z2直丝弓矫治器的错畸形患者于牵拉尖牙远移靠拢第二双尖牙前后,使用原子力显微镜观察和测量在口内环境中的0.018″×0.025″方形不锈钢片段弓托槽滑过段弓丝四个面滑过前后、未滑过的磨牙前段弓丝四个面表面形貌的结构变化。测量参数选用三维参数(Sz、Sp、Sv、St、Sdc),并于spss13.0统计学软件中对数据进行统计分析,作滑过前后、临床组内、临床组间结构及变化量的比较。 结果:(1)临床中自锁组尖牙远移牵引力约120g/侧,移动速度约为0.96mm/月;直丝组约为150g/侧,移动速度约为0.72mm/月。(2)原子力显微镜扫描下同一厂家、品牌的弓丝结构差异无显著性。(3)托槽滑动过后Z2直丝组弓丝四个面均变粗糙;自锁组上下宽面弓丝结构差异有显著性,均变光滑;外窄面结构差异无显著性,弓丝表面无明显变化;内窄面结构差异有显著性,变得略粗糙。(4)自锁组尖牙滑过后尖牙滑过段弓丝与磨牙前段弓丝结构变化量四个面均有显著性差异,而Z2直丝组仅内窄面有显著性差异,尖牙滑过段内窄面比磨牙前段变得更粗糙,余三个面变化量差异无显著性,变粗糙程度基本相近。(5)口内环境下,两组无托槽滑过的磨牙前段弓丝在尖牙滑动时间段内均变粗糙;变粗糙程度内外窄面差异有显著性,以Z2直丝组为严重,上下宽面无显著性差异,两组变粗糙程度相近。 结论:1、同一厂家、品牌的方形弓丝表面结构基本相同。2、与Z2直丝弓组相比,自锁组托槽滑过弓丝后,弓丝表面结构损坏小,多变光滑。尖牙远移摩擦力小,牵引力较小,牙移动较快。3、于口内自然环境下,固定矫治器的方形不锈钢弓丝,即使无托槽滑过,随治疗时间加长,表面会变粗糙,,应及时更换。
[Abstract]:Objective: to investigate the effect of self-locking bracket and common straight wire bracket on the surface structure of 0.018 "脳 0.025" square stainless steel arch wire during tooth movement.Methods: the malformed patients wearing Damon-Q self-locking bracket and Z2 straight wire appliance were treated with the second bicuspid teeth before and after pulling the canine teeth.Atomic force microscopy (AFM) was used to observe and measure the structural changes of the 0.018 "脳 0.025" square stainless steel segment arch bracket in the oral environment before and after sliding through the four facets of the segmental arch wire, and the surface morphology of the four surfaces of the unskid anterior segment arch wire was observed and measured.The measurement parameters were selected as three dimensional parameters, and the data were statistically analyzed by spss13.0 software. The structure and variation of the data were compared between the clinical groups before and after sliding and in the clinical group.Results (1) in the self-locking group, the distal distraction force of the canine teeth was about the 120g/ side, and the moving speed was about 0.96mm/ month, and the straight wire group was about the 150g/ side and the moving speed was about 0.72mm/ month. 2) the same manufacturer was scanned by atomic force microscope.There was no significant difference in the arch wire structure of the brand. (3) after sliding the brackets, the four facets of the Z2 straight wire group all became rough, the difference of the upper and lower wide plane arch wire structure of the self-locking group was significant, all of them became smooth, and the difference of the outer narrow plane structure was not significant.There was no obvious change in the surface of arch wire, and there was significant difference in the structure of the inner narrow surface, which became slightly rough. 4) in the self-locking group, there were significant differences in the changes of the structure of the anterior segment of the arch wire between the canine and the anterior segment of the molar after the canine slip.In Z2 straight wire group, there was significant difference only in the inner narrow surface. The narrow surface of the canine was rougher than that of the anterior molar, but there was no significant difference in the change of the other three surfaces. The degree of coarseness was similar to that in the intraoral environment.The coarseness degree of the anterior arch wire in the anterior segment of the molars without brackets slid during the period of the canine sliding had significant difference between the two groups, especially in the Z2 straight wire group, there was no significant difference in the upper and lower width, and the degree of coarseness was similar between the two groups.ConclusionThe surface structure of the square arch wire of the same manufacturer is basically the same as that of the Z2 straight wire group. Compared with the Z2 straight wire group, the surface structure of the arch wire in the self-locking group after sliding over the bow wire is less damaged and changeable and smooth than that in the Z2 straight wire group.The distal movement of the canine teeth is small, the traction force is small, the tooth moves faster. 3. In the natural environment of the mouth, the square stainless steel arch wire of fixing the appliance, even if no bracket slips, the surface will become rougher with the treatment time, and should be replaced in time.
