趋化因子CCL7在口腔白斑及口腔鳞癌组织中的表达及意义

【摘要】:WHO将口腔白斑归入癌前病变的范畴。而口腔鳞状细胞癌是我国口腔颌面部最为常见的恶性肿瘤。鳞癌局部侵袭力强,常向区域淋巴结转移,晚期容易发生远处转移。目前它的主要有效的治疗方法仍以手术切除为主,,即使辅助放射治疗、化学药物治疗等综合序列治疗,它的预后效果往往不佳,生存率低。因此,为了口腔鳞癌的早期预防、发现及诊断,提高口腔鳞癌患者的生存质量及存活率,深入探讨口腔鳞癌的发生发展机制并针对性地采取个体化综合序列治疗显得尤为重要。 大量的研究证明,各种免疫因子、趋化因子及其受体在肿瘤的发生发展过程中起着及其重要的作用。CCL7是趋化因子CC家族中的一员,其在募集单核细胞、肿瘤转移等生物学效应中起着重要作用,有研究表明该因子在宫颈癌、胰腺癌、胃癌、结直肠癌等恶性肿瘤中与肿瘤的发展转移有关。CCL7在口腔鳞状细胞癌中表达情况的研究较少,本研究采用免疫组化技术,对口腔白斑、口腔鳞癌中CCL7的表达进行初步研究,结合临床资料进行分析,来探讨CCL7在它们当中的表达情况。 目的 检测CCL7在口腔白斑和口腔鳞癌患者肿瘤组织中的表达情况,分析与临床病理参数的关系,初步探讨CCL7在口腔癌前病变恶变及口腔鳞癌发生发展中的作用。 材料和方法 1.样本资料:18例口腔白斑组织,其中12例单纯增生型白斑,6例中或中重度增生型白斑;35例口腔鳞癌肿瘤组织,12例高分化型鳞癌,13例中或中低分化型鳞癌,10例中或中低分化且伴有颈部淋巴结转移型鳞癌;11例口腔正常黏膜组织; 2.实验方法:采用免疫组织化学染色(SABC)的方法进行检测CCL7的表达; 3.统计方法:所得实验数据用SPSS19.0软件进行分析,各组间免疫组化的阳性率及临床病理分级等计数资料的统计分析比较采用X2检验,平均光密度值采用t检验,p0.05为差异具有统计学意义。 结果 口腔鳞癌患者组织中CCL7的表达高于口腔白斑组织及正常口腔黏膜组织,差异具有统计学意义(p0.05)。口腔鳞癌组织中临床分期晚期伴有颈部淋巴结转移的组织中CCL7的表达高于临床早期无淋巴结转移组织,p0.05,差异具有统计学意义。而中或中重度增生的白斑组织中CCL7的表达高于单纯增生的白斑组织及正常口腔黏膜组织,p0.05,差异具有统计学意义。 结论 CCL7在中重度异常增生的白斑组织及口腔鳞癌患者肿瘤组织中的高表达,且与临床分期及颈部淋巴结转移相关,提示CCL7可能在口腔癌前病变恶变及口腔鳞癌的发生发展过程中起着一定的作用。
[Abstract]:WHO classified oral leukoplakia as precancerous lesions.Oral squamous cell carcinoma (OSCC) is the most common malignant tumor in oral and maxillofacial region in China.Squamous cell carcinoma has strong local invasiveness and often metastases to regional lymph nodes, and is prone to distant metastasis in late stage.At present, its main effective treatment method is still surgical resection, even though the adjuvant radiotherapy, chemotherapeutic therapy and other comprehensive sequence therapy, its prognosis is often poor, the survival rate is low.Therefore, in order to prevent, detect and diagnose oral squamous cell carcinoma early, improve the quality of life and survival rate of oral squamous cell carcinoma patients, deeply explore the development mechanism of oral squamous cell carcinoma and take individualized comprehensive sequence therapy.A large number of studies have proved that all kinds of immune factors, chemokines and their receptors play an extremely important role in tumorigenesis and progression. CCL7 is a member of the CC family of chemokines, which is recruiting monocytes.Tumor metastasis and other biological effects play an important role, some studies have shown that the factor in cervical cancer, pancreatic cancer, gastric cancer,There are few studies on the expression of CCL7 in oral squamous cell carcinoma (OSCC) in colorectal cancer and other malignant tumors. The expression of CCL7 in oral leukoplakia and oral squamous cell carcinoma (OSCC) was studied by immunohistochemical technique.To explore the expression of CCL7 in them by analyzing the clinical data.PurposeTo detect the expression of CCL7 in oral leukoplakia and oral squamous cell carcinoma (OSCC), to analyze the relationship between CCL7 and clinicopathological parameters, and to explore the role of CCL7 in the development of oral squamous cell carcinoma (OSCC) and precancerous lesion.Materials and methods1.Sample data: 18 cases of oral leukoplakia,12 cases of simple proliferative leukoplakia 6 cases of moderate or severe proliferative leukoplakia 35 cases of oral squamous cell carcinoma tumor 12 cases of highly differentiated squamous cell carcinoma 13 cases of middle or low differentiated squamous cell carcinoma 10 cases of middle or low differentiation with cervical lymph node metastasis11 cases of normal oral mucosa;2.Methods: immunohistochemical staining was used to detect the expression of CCL7.3.Methods: the experimental data were analyzed by SPSS19.0 software. The positive rate of immunohistochemistry and the clinical pathological grade were compared by X2 test.The average optical density was significantly different by t test (p 0.05).ResultThe expression of CCL7 in oral squamous cell carcinoma was significantly higher than that in oral leukoplakia and normal oral mucosa.The expression of CCL7 in oral squamous cell carcinoma (OSCC) with advanced stage and cervical lymph node metastasis was significantly higher than that in early stage without lymph node metastasis (p0.05).The expression of CCL7 in moderate or severe hyperplastic leukoplakia tissues was higher than that in simple proliferative leukoplakia tissues and normal oral mucosa tissues (P 0.05).ConclusionThe high expression of CCL7 in leukoplakia and oral squamous cell carcinoma patients with moderate or severe dysplasia was associated with clinical stage and cervical lymph node metastasis.The results suggest that CCL7 may play an important role in the development of oral squamous cell carcinoma and precancerous lesion.
