乌鲁木齐市汉族青少年颈椎骨龄与第二磨牙钙化程度的相关性研究

【摘要】:目的:研究乌鲁木齐市汉族青少年颈椎骨龄与下颌第二磨牙钙化程度之间的相关性,探讨这两项指标在口腔正畸临床中的应用价值,为正畸临床诊断并预测生长发育阶段提供理论依据。方法:对乌鲁木齐市300名9~15岁汉族青少年(128名男性,172名女性)进行曲面断层片和头颅侧位片的观察测量,应用Hassel和Farman的改良颈椎分析法评估患者颈椎骨龄和Demirjian法对左下第二磨牙进行发育和钙化阶段评估,运用统计学分析方法探讨两者之间的相关性。结果:(1)通过统计分析得出颈椎骨龄与下颌第二磨牙钙化程度之间的相关性呈高度相关(r=0.718,P0.01),男性颈椎骨龄与下颌第二磨牙钙化程度呈高度相关(r=0.780,P0.01),密切程度高于女性(r=0.661,P0.01),结果具有统计学意义。(2)在整体样本中,用牙齿钙化D阶段诊断生长发育起始期阳性似然比为18.49,用牙齿钙化H阶段诊断生长发育成熟期阳性似然比为8.48,在女性组中,用牙齿钙化D阶段诊断生长发育起始期阳性似然比为41,牙齿钙化H阶段诊断生长发育成熟期阳性似然比为7.07,在男性组中,用牙齿钙化D阶段诊断生长发育起始期阳性似然比为10.64,用牙齿钙化H阶段诊断生长发育成熟期阳性似然比为10.92,牙齿钙化F阶段诊断生长发育过渡期阳性似然比为6.35。结论:乌鲁木齐市汉族青少年颈椎骨龄与下颌第二磨牙钙化程度均显著相关,对于第二磨牙钙化程度作为判断青少年生长发育阶段时:女性、男性牙齿钙化D阶段均可以作为生长发育的起始阶段诊断工具,男性牙齿钙化H阶段可以作为生长发育的成熟阶段的诊断工具;女性牙齿钙化H阶段和男性牙齿钙化F阶段可以分别作为生长发育的成熟阶段和过渡阶段的辅助诊断工具,需综合年龄、骨骼成熟度和牙齿钙化程度三种方法共同评估患者所处的生长发育阶段。
[Abstract]:Objective: to study the correlation between bone age of cervical vertebrae and calcification degree of mandibular second molar in Urumqi Han nationality adolescents, and to explore the clinical value of these two indexes in orthodontic treatment.To provide theoretical basis for clinical diagnosis of orthodontics and prediction of growth and development stage.Methods: surface tomography and lateral cephalograms were performed on 300 9- 15 year old Han nationality adolescents (128 males and 172 females) in Urumqi.Hassel and Farman were used to evaluate the bone age of cervical vertebrae and Demirjian method to evaluate the development and calcification stages of the left lower molars. The correlation between the two methods was studied by statistical analysis.Results by statistical analysis, it was found that there was a high correlation between cervical spine bone age and the degree of calcification of mandibular second molar, and there was a high correlation between bone age of male cervical vertebra and degree of calcification of mandibular second molar, which was higher than that of female.The results were statistically significant in the overall sample.The positive likelihood ratio was 18.49 for D stage of tooth calcification and 8.48 for H stage of tooth calcification.The positive likelihood ratio of dental calcification in D stage was 41 in the initial stage of growth and development, and 7.07 in the H stage of dental calcification.The positive likelihood ratio of tooth calcification in D stage was 10.64, that in H stage of tooth calcification was 10.92 in mature stage, and 6.35 in F stage of tooth calcification.Conclusion: the bone age of cervical vertebrae of Han nationality adolescents in Urumqi is significantly correlated with the degree of calcification of mandibular second molars.The D stage of male tooth calcification can be used as a diagnostic tool for the initial stage of growth and development, and the H stage of male tooth calcification can be used as a diagnostic tool for the mature stage of growth and development.The female dental calcification H stage and the male tooth calcification F stage can be used as auxiliary diagnostic tools for the mature and transitional stages of growth and development, respectively.Bone maturity and dental calcification are three methods to assess the growth and development stages of patients.
