慢性间歇性低氧条件下大鼠龈沟液中牙龈卟啉单胞菌的研究

【摘要】:目的:本实验的目的是模拟阻塞型睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)病人的间歇性低氧条件,来探讨大鼠龈沟液中牙龈卟啉单胞菌的变化。方法:将32只雄性SD大鼠随机分成四组:常氧对照组(A组)、常氧牙周炎组(B组)、间歇性低氧组(C组)、间歇性低氧合并牙周炎组(D组)。采用正畸结扎丝结扎双侧上颌第二磨牙颈部和高糖饮食方法建立牙周炎模型,常氧组和低氧组分别在常氧条件和模拟阻塞性睡眠呼吸暂停低通气综合征条件下饲养,8周后,对各组大鼠进行牙周检查,记录牙周各项临床指标,同时采集每只大鼠上颌右侧第二磨牙龈沟液标本,然后用空气栓塞法处死各组大鼠,取出上颌骨,观察牙槽骨吸收情况。采用实时荧光定量PCR方法检测龈沟液中牙龈卟啉单胞菌的变化。结果:1.建模第8周,B组和D组大鼠实验牙食物堆积,牙龈红肿,探诊出血,有深的牙周袋形成,而A组和C组大鼠实验牙牙周组织变化不大,牙龈基本正常。说明牙周炎模型建立成功。2.四组大鼠牙周临床检查:D组大鼠牙周临床检查指标高于其他各组,差异有统计学意义(p0.05);B组大鼠临床检查指标高于A组,差异有统计学意义(p0.05)。C组大鼠临床检查指标高于A组,差异有统计学意义(p0.05)。3.D组牙槽骨较其他各组吸收明显。4.所有的大鼠均能检出牙龈卟啉单胞菌。D组牙龈卟啉单胞菌的量明显高于其他各组,差异有统计学意义(p0.05);B组牙龈卟啉单胞菌的量高于A组、C组两组,差异有统计学意义(p0.05)。而A组和C组相比,牙龈卟啉单胞菌的量差异无统计学意义。结论:1.本实验成功地建立慢性间歇性低氧条件下大鼠牙周炎模型。2.牙龈卟啉单胞菌为口腔龈沟液中的常驻菌。慢性间歇性低氧条件下龈沟液中牙龈卟啉单胞菌数量增加,加重大鼠牙周炎的病变程度。
[Abstract]:Objective: to investigate the changes of porphyromonas gingivalis in rat gingival crevicular fluid (GCF) by simulating intermittent hypoxia in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). Methods: Thirty-two male Sprague-Dawley rats were randomly divided into four groups: group A (normoxic control group), group B (normoxic periodontitis group), group C (intermittent hypoxia group) and group D (intermittent hypoxia combined with periodontitis group). The periodontitis model was established by orthodontic ligation of bilateral maxillary second molar neck and high glucose diet. The rats in normoxic and hypoxic groups were fed with obstructive sleep apnea hypopnea syndrome (OSAS) for 8 weeks. Periodontal examination was performed on each group of rats, and the clinical indexes of periodontal were recorded. At the same time, the gingival crevicular fluid samples of the second grinding on the right side of the maxilla were collected, then the rats were killed by air embolization, the maxillary bone was removed and the alveolar bone resorption was observed. The changes of porphyromonas gingivalis in gingival crevicular fluid were detected by real-time fluorescent quantitative PCR. The result is 1: 1. In the 8th week of modeling, the experimental teeth of group B and group D had accumulated food, gingival redness, bleeding and deep periodontal pocket formation, but the periodontal tissue of group A and group C had little change, and the gingiva was basically normal. The results showed that the model of periodontitis was established successfully. The periodontal clinical examination in group D was higher than that in other groups, and the difference was statistically significant. The clinical examination index of group B was higher than that of group A, and the difference of clinical examination index of group C was higher than that of group A. The difference was statistically significant (P 0.05). 3.The alveolar bone in group D was significantly higher than that in other groups. All rats were able to detect the amount of porphyromonas gingivalis in group D, which was significantly higher than that in other groups, and the difference was statistically significant in group B than in group A and group C, and the difference was statistically significant (P 0.05). There was no significant difference in the quantity of porphyromonas gingivalis between group A and group C. Conclusion 1. In this experiment, the rat periodontitis model was successfully established under chronic intermittent hypoxia. Porphyromonas gingivalis is a resident bacteria in oral gingival crevicular fluid. Under chronic intermittent hypoxia, the quantity of porphyromonas gingivalis in gingival crevicular fluid increased, which aggravated the severity of periodontitis in rats.
