老年慢性牙周炎患者龈沟液基质金属蛋白酶-1、基质金属蛋白酶组织抑制物-1水平与龈下刮治和根面平整术疗效的相关性

【摘要】：目的探究老年慢性牙周炎患者龈沟液(GCF)中基质金属蛋白酶(MMP)-1和MMP组织抑制物(TIMP)-1水平与龈下刮治和根面平整术效果和牙周组织状况的关系。方法 53例龈下刮治和根面平整术治疗的老年慢性牙周炎患者为观察组,49例行健康检查的老年人为对照组。对比治疗前后两组牙周指标菌斑指数(PI)、探针出血(BOP)、探针深度(PD)、附着丧失(AL)及GCF中MMP-1和TIMP-1水平差异变化;分析观察组GCF中MMP-1、TIMP-1与PI的相关性。结果两组治疗前牙周指标及GCF中MMP-1、TIMP-1水平差异均显著,且治疗后观察组牙周指标及GCF中MMP-1、TIMP-1水平与对照组及治疗前差异显著,观察组GCF中MMP-1(r=0.493,P0.001)及TIMP-1(r=0.515,P0.001)与PI呈正相关。结论老年慢性牙周炎患者经龈下刮治和根面平整术治疗后效果显著,牙周组织现状好转改善,GCF中MMP-1、TIMP-1水平下降明显,炎症反应逐渐消失。
[Abstract]:Objective to investigate the relationship between the levels of matrix metalloproteinase-1 (MMP-1) and tissue inhibitor of MMP (TIMP- 1) in gingival crevicular fluid (GCF) of elderly patients with chronic periodontitis and the effect of subgingival curettage, root surface leveling and periodontal tissue status. Methods 53 cases of senile chronic periodontitis treated by subgingival curettage and root surface leveling were treated as control group (n = 49). The levels of MMP-1 and TIMP-1 in GCF and MMP-1 and TIMP-1 in GCF were compared before and after treatment, and the correlation between TIMP-1 and Pi in GCF was analyzed. Results there were significant differences between the two groups in the periodontal indexes and the levels of MMP-1 and TIMP-1 in GCF, and the levels of MMP-1 and TIMP-1 in GCF in the observation group were significantly higher than those in the control group and before treatment. There was a positive correlation between the levels of MMP-1 and TIMP-1p0.001 in GCF and TIMP-1r0.515P0.001 in the observation group. Conclusion the effect of subgingival curettage and root surface leveling in elderly patients with chronic periodontitis was remarkable. The improvement of periodontal tissue and improvement of the level of MMP-1 and TIMP-1 in GCF decreased significantly, and the inflammatory reaction gradually disappeared.
