Bicon种植体冠根比对后牙区骨组织及软组织影响的短期临床效果观察

【摘要】:目的:评估Bicon短种植体(6-8mm)在后牙区骨量不足患者的种植体冠根比(crowntoimplantratio,C/I)的短期临床效果。观察并记录种植体冠根比对近、远中边缘骨高度和骨水平、以及冠根比对软组织的影响。方法:选取2012年1月-2016年12月,山东大学种植科患者共10例,后牙区植入Bicon种植体共13枚,采用X线平行投照技术,观察修复体戴入当天和使用3年之后,修复体的冠根比(C/I)与种植体近、远中边缘骨高度变化的关系。临床检查观察种植体软组织的变化。将种植修复体按冠根比分为两组:组1:C/I2.0,组2:C/I"g2.0。计算提供在总样本中的两组数据分布夏皮罗计算差异、皮尔森计算相关、Kendall秩相关系数,显示种植体冠长度与软组织参数相关性。结果:13枚Bicon种植体,负重后种植体的平均C/I比为(1.854±0.292),范围从1.561到2.146不等。统计学分析种植体冠根比组1(C/I2.0),组2(C/I"g2.0)之间无差异。组1与组2在观察期间修复体负载正常,种植体近、远中边缘骨高度、牙槽骨水平无明显差异。牙槽骨吸收量组1(-0.24±0.98),组2(-0.12±0.28)为负值说明两组间无牙槽骨丢失。测量患者平均mPI值和mSBI值得出结果P0.05表明种植体的高冠根比可能导致种植体周围软组织炎症。结论:Bicon种植体有较常规种植体较高的冠根比,经过3年的功能负荷评估中,种植体边缘骨及骨水平丢失的风险没有增加,在后牙骨量不足的患者有很好的临床应用价值。高冠根比对于种植体软组织,可有软组织炎症,应定期做口腔检查及种植牙的维护。
[Abstract]:Objective: to evaluate the short-term clinical effect of Bicon short implant (6-8mm) in patients with low bone mass in posterior teeth. The bone height and bone level of the distal margin and the influence of the ratio of crown to root on soft tissue were observed and recorded. Methods: from January 2012 to December 2016, 10 patients were enrolled in the Department of Implant, Shandong University, and 13 Bicon implants were implanted in the posterior teeth. The relationship between the ratio of crown to root (C / I) and the height of the proximal and distal margin of the implant. The changes of implant soft tissue were observed by clinical examination. The implants were divided into two groups: group 1: C / I 2.0, group 2:C/I "g 2.0." Shapiro calculated the differences between the two groups of data distribution in the total sample. Pearson calculated the correlation Kendall rank correlation coefficient and showed the correlation between implant crown length and soft tissue parameters. Results the average C / I ratio of 13 Bicon implants was (1.854 卤0.292), ranging from 1.561 to 2.146. There was no significant difference in implant crown to root ratio between group 1 (C/I2.0) and group 2 (C / I "g 2.0). There was no significant difference between group 1 and group 2 in the level of alveolar bone and the height of distal marginal bone. The alveolar bone resorption was negative in group 1 (-0.24 卤0.98) and group 2 (-0.12 卤0.28), indicating that there was no alveolar bone loss between the two groups. The measurement of mean mPI value and mSBI were worth the results P0.05 indicated that the high root ratio of implant may lead to soft tissue inflammation around the implant. Conclusion the ratio of crown to root is higher than that of conventional implants. After 3 years of functional load assessment, the risk of loss of marginal bone and bone level is not increased, and it has a good clinical application value in patients with insufficient posterior bone mass. High crown root ratio for implant soft tissue may have soft tissue inflammation, should be regular oral examination and dental implant maintenance.
