上颌窦底外提升同期种植体植入术在修复上颌后牙区严重骨量不足中的应用

【摘要】：目的针对上颌窦底剩余牙槽骨高度(residual bone height,RBH)2~3 mm的患者行上颌窦底外提升同期种植体植入术的病例研究报道较少。文中旨在评价应用上颌窦底外提升同期种植体植入术修复上颌后牙区严重骨量不足病例的临床效果。方法回顾性分析2012年1月至2013年1月在常州市口腔医院种植中心行上颌窦底外提升同期种植体植入术的42例患者的临床资料,共植入85颗种植体。术前锥形束计算机体层扫描测量上颌后牙种植区剩余牙槽骨高度,其中RBH为2~3 mm的53枚种植体为试验组,32颗RBH在4~5 mm的种植体为对照组。85枚种植体中单颗种植体为单冠组(19颗),≥2颗种植体为桥体组(66颗)。术中采用超声骨刀行上颌窦底外提升同期种植体植入术,并植入自体骨+人工骨粉混合物行骨再生术,修复前测量种植体稳定性系数(Implant stability Quotient,ISQ),术后2年测量牙槽嵴顶至上颌窦底的高度,随访24个月,观察临床疗效。结果试验组和对照组术后24个月骨提升高度、ISQ比较差异有统计学意义[(7.755±0.376)vs(6.658±0.415)mm;(74.444±1.462)vs(73.058±1.477),P0.05]。logistic回归分析显示术后24个月骨提升高度的OR值为1.248(95%CI:0.242~6.427)。种植体提升高度对种植体的稳定性的影响无统计学意义(P=0.149)。试验组的种植体存留率与对照组比较,差异无统计学意义(98.11%vs 100%,P=0.953)。单冠组种植体存留率与桥体组比较,差异无统计学意义(94.74%vs 100%,P=0.189)。术后24个月复查,试验组、对照组种植体周围骨均整合良好并成功负载,种植体颈部骨少量吸收,种植体稳定。结论上颌窦外提升同期种植体植入术应用于上颌后牙区严重骨量不足的种植修复病例临床效果满意。
[Abstract]:Objective to study the patients with 2mm (residual bone height of residual alveolar bone in the maxillary sinus floor undergoing simultaneous implant implantation with exasperation of the maxillary sinus floor. The purpose of this study was to evaluate the clinical effect of simultaneous implant implantation in the repair of severe bone deficiency in maxillary posterior teeth. Methods from January 2012 to January 2013, the clinical data of 42 patients who underwent the simultaneous implant implantation in the implant center of Changzhou Stomatological Hospital were analyzed retrospectively. A total of 85 implants were implanted. The height of residual alveolar bone in maxillary posterior dental implant was measured by computer tomography of conical bundle before operation.
