牙周病非手术治疗研究进展附病例报告

【摘要】:背景:牙周病是口腔中较为常见的疾病。它是一种主要由牙周致病菌所引起,导致牙齿支持组织发生炎症和进行性破坏的感染性疾病。菌斑生物膜是牙周病的始动因子。破坏龈上和(或)龈下菌斑生物膜的结构,并将其与牙面和(或)根面的牙结石彻底去除,是牙周病基础治疗的主要目标。目前,牙周基础治疗是临床上应用最广和最为有效的治疗牙周病的方法。为了提高牙周基础治疗的临床疗效,减少传统牙周非手术治疗的临床局限性及缺点,避免某些重度牙周炎患者对牙周手术治疗干预的需要,近些年来,各种新的行之有效的治疗策略被提出。方法:通过对牙周病非手术治疗的相关内容进行广泛性的文献回顾(包含综述、系统评价和随机对照试验等),介绍了传统牙周病非手术治疗的现状,并从传统牙周治疗器械的改良、牙周病治疗新技术的开展和牙周病新治疗方案的应用等方面,对其研究进展进行相应描述和评价。结果:传统的牙周非手术治疗存在着一定的局限性。药物治疗可作为牙周基础治疗的补充。传统牙周治疗器械和治疗方式的改良,不单使牙周基础治疗取得更好的疗效,而且患者能够获得更好的治疗体验。牙周基础治疗辅以牙周内镜、激光和光动力等新技术治疗牙周病,其效果优于传统的治疗方法。甘氨酸龈下喷砂可作为牙周维护期去除龈下菌斑和牙石的另一种有效治疗手段。结论:不论在牙周非手术治疗中使用何种治疗器械或者采用何种治疗方案,牙周非手术治疗都是治疗牙周疾病的有效治疗方法。虽然先进的治疗技术和设备等在不断发展更新,但是牙周非手术治疗仍然是治疗牙周病的金标准。
[Abstract]:Background: periodontal disease is a common disease in the oral cavity. It is an infectious disease mainly caused by periodontal pathogenic bacteria, which causes inflammation and progressive destruction of dental supporting tissue. Plaque biofilm is the initiator of periodontal disease. Destroying the structure of the supragingival and / or subgingival plaque biofilm and removing the calculus from the tooth surface and / or root surface is the main goal of the basic treatment of periodontitis. At present, periodontal basic therapy is the most widely used and most effective method in the treatment of periodontal disease. In order to improve the clinical effect of periodontal basic treatment, reduce the clinical limitations and shortcomings of traditional periodontal non-surgical treatment, and avoid the need of some severe periodontitis patients to intervene in periodontal surgery, in recent years, Various new and effective treatment strategies have been proposed. Methods: the current situation of non-operative treatment of periodontal disease was reviewed by reviewing the literature (including review, systematic evaluation and randomized controlled trial). The research progress was described and evaluated from the aspects of the improvement of traditional periodontal treatment instruments, the development of new techniques for periodontal disease treatment and the application of new treatment schemes for periodontal diseases. Results: the traditional periodontal non-operative treatment has some limitations. Drug therapy can be used as a supplement to basic periodontal therapy. The improvement of the traditional periodontal treatment instruments and treatment methods not only makes periodontal basic treatment obtain better curative effect, but also the patients can get better treatment experience. Periodontal basic therapy combined with periodontal endoscopy, laser and photodynamic therapy is more effective than traditional methods. Glycine subgingival blasting can be used as another effective treatment for removal of subgingival plaque and odontolith during periodontal maintenance. Conclusion: periodontal non-surgical treatment is an effective treatment for periodontal diseases, no matter what instruments or treatments are used in periodontal non-surgical treatment. Although advanced treatment technology and equipment are developing, periodontal non-surgical treatment is still the gold standard for periodontal disease.
