翼外肌在髁突矢状骨折愈合中对髁突应力分布作用的三维有限元研究

【摘要】:目的从生物力学角度分析髁突矢状骨折(SFMC)愈合进程中的应力分布情况,为SFMC临床治疗提供依据。方法以三维有限元法建立包含双侧颞颌关节(TMJ)的下颌骨有限元模型,设左侧为骨折侧,用0.1mm的薄层模拟骨折裂隙,建立髁突矢状骨折模型,计算骨折不同愈合阶段下颌骨整体的应力分布状况和骨折侧髁突、关节盘表面应力分布、骨折断端位移的状况。结果翼外肌牵拉时,Ⅱ型骨折髁突游离端在骨折当天到自然愈合的第4周应力和位移都增大,各观测组均于第4~12周趋于平稳,髁突残端在自然愈合过程中的应力和位移较平稳;关节盘残端骨折当天到自然愈合的第4周应力减小,第4~12周趋于平稳,位移趋于平稳;关节盘游离端骨折当天到自然愈合的第4周应力和位移都增大,第4~12周趋于平稳。切断翼外肌后,Ⅱ型骨折髁突游离端在骨折当天到自然愈合的第4周应力和位移都增大,第4~12周趋于平稳,髁突残端在骨折当天到愈合第4周的应力和位移减小,第4~12周趋于平稳;关节盘残端骨折当天到自然愈合的第4周应力减小,第4~12周趋于平稳,位移趋于平稳;关节盘游离端骨折当天到自然愈合的第4周应力和位移都增大,第4~12周趋于平稳。结论翼外肌在髁突矢状骨折愈合中对髁突应力和位移有影响,在髁突矢状骨折治疗时应保留翼外肌。
[Abstract]:Objective to analyze the stress distribution in the healing process of condylar sagittal fracture (SFMC) from the biomechanical point of view so as to provide evidence for the clinical treatment of SFMC. Methods the finite element model of mandible including bilateral temporomandibular joint (TMJ) was established by three dimensional finite element method. The left side was used as the fracture side, and the condylar sagittal fracture model was established by simulating fracture with thin layer of 0.1mm. The stress distribution of the mandible at different healing stages, the stress distribution of the lateral condyle, the surface stress of the articular disc, and the displacement of the broken end of the fracture were calculated.
