非平衡等离子体对感染根管细菌生物膜影响的实验研究

【摘要】:目的:大量研究资料表明,非平衡等离子体具有良好的灭菌效果。由于其气体温度接近室温,对机体组织无明显损害,活性成分存活周期短而无毒性物质残留,能够到达微小不规则的部位,其在医学领域的应用受到了越来越多的关注。本课题组的早期实验研究证实,非平衡等离子体能够有效地杀灭粪肠球菌生物膜,其灭菌效果与常规根管冲洗药物相同。因此,本课题组将常规根管冲洗药物洗必泰与非平衡等离子体相结合,形成CHX等离子体,并通过分别应用根管消毒封闭、非平衡等离子体和CHX等离子体三种治疗措施,综合评估其对感染根管细菌生物膜的影响。 方法:粪肠球菌或龈下菌斑悬液接种于无菌盖玻片、牛牙本质块、经过预备的单根管离体牙及牙本质小管内,于37℃厌氧培养,在体外建立感染根管细菌生物膜模型,分别应用根管消毒封闭、非平衡等离子体和CHX等离子体三种治疗措施进行灭菌处理。随后,细菌活性荧光染料对处理后的细菌生物膜进行染色,立即置于激光扫描共聚焦显微镜下观察,并进行三维重建,利用软件Imaris计算得出生物膜细菌的死菌比例,从而分析这三种治疗措施的灭菌效果。 结果:经过三维重建后的感染根管细菌生物膜图片显示,根管消毒封闭、非平衡等离子体和CHX等离子体三种治疗措施都能够有效地杀灭绝大多数生物膜细菌;CHX等离子体可清除更多的细菌生物膜,灭菌效果明显优于常规根管冲洗药物2%洗必泰和非平衡等离子体(P0.05);延长作用时间,更多的生物膜细菌被灭菌手段灭活(P0.05);相较于成熟的细菌生物膜来说,年轻的生物膜细菌对灭菌处理的抵抗性更弱,更容易被清除(P0.05)。 结论:CHX等离子体对体外的细菌生物膜具有良好的灭菌效果。在牙髓根尖周疾病治疗过程中,其可以作为一种全新的辅助手段来控制细菌感染,以增加根管治疗成功率,提高患牙保存率。CHX等离子体的生物相容性和对细菌生物膜的灭菌作用机理仍需进一步研究。

Because the gas temperature is close to room temperature, there is no obvious damage to the body tissue, the survival period of active components is short and the residue of nontoxic substances can reach small and irregular parts, its application in the field of medicine has been paid more and more attention. The early experiments of our group have proved that non-equilibrium plasma can effectively kill the biofilm of Enterococcus faecalis, and its sterilization effect is the same as that of conventional root canal washing drugs. Therefore, our group combined chlorhexidine with non-equilibrium plasma to form CHX plasma, which was treated by root canal disinfection, non-equilibrium plasma and CHX plasma, respectively. The effects of biofilm on bacterial biofilm of infected root canal were comprehensively evaluated.
