平均生长型青少年不同矢状骨面型切牙与磨牙倾斜度研究

【摘要】:目的:通过对平均生长型青少年三种矢状骨面型治疗前头颅定位侧位片进行测量,比较三种不同矢状骨面型切牙唇舌向和第一恒磨牙近远中倾斜度代偿特点及差异,为临床矫治设计、支抗预备及正畸术后长期稳定提供参考依据。方法:依据纳入标准与面部生长型分型标准从正畸患者中选取平均生长型青少年120例,按ANB角行矢状向分型:Ⅰ类骨面型(ANB角0°-5°),Ⅱ类骨面型(ANB角≥5°),Ⅲ类骨面型(ANB角≤0°)三组每组各40例,使用13个角度作为切牙及第一恒磨牙倾斜度的测量指标,三组进行单因素方差分析及两两比较用LSD-t检验。结果:1.不同矢状骨面型切牙倾斜度测量结果:三组不同的矢状骨面型中,∠U1-SN、∠U1-FH、∠U1-PP的测量值差异均无统计学意义;∠L1-MP的测量值三组间的差异有统计学意义,骨性Ⅲ类∠L1-MP的数值小于Ⅰ类、Ⅱ类,骨性Ⅰ类、Ⅱ类之间无区别;∠U1-L1的测量值三组间差异有统计学意义,骨性Ⅲ类∠U1-L1的数值大于Ⅰ类、Ⅱ类,骨性Ⅰ类、Ⅱ类之间无区别。2.不同矢状骨面型第一恒磨牙倾斜度测量结果:∠U6-SN、∠U6-FH、∠U6-PP、∠L6-SN均是骨性Ⅲ类组大于骨性Ⅰ类、Ⅱ类组,∠L6-MP骨性Ⅲ类组小于Ⅰ类、Ⅱ类组,差异均有统计学意义;∠U6-L6的测量值三组间差异无统计学意义。下颌切牙与磨牙倾斜度的测量值均是骨性Ⅲ类组的小于Ⅰ类、Ⅱ类,提示骨性Ⅲ类下切牙、下颌第一恒磨牙代偿性倾斜一致;骨性Ⅲ类下切牙较Ⅰ类代偿变化角度为9.24°,下颌第一恒磨牙较Ⅰ类代偿变化角度为4.11°,提示骨性Ⅲ类下切牙的代偿性比下磨牙的明显。结论:1.随着ANB角的减小,平均生长型青少年下颌切牙舌向倾斜,上颌第一恒磨牙近中倾斜,下颌第一恒磨牙远中倾斜,上颌切牙唇舌向倾斜度在三种骨面型中无差别。2.平均生长型青少年骨性Ⅲ类上颌第一恒磨牙近中倾斜代偿,下颌第一恒磨牙远中倾斜代偿,下切牙舌向倾斜代偿;下切牙与磨牙代偿倾斜趋势一致,下颌切牙代偿较磨牙明显。3.骨性Ⅱ类与Ⅰ类比较,上切牙趋于舌向倾斜,上颌第一恒磨牙趋于远中倾斜,下切牙趋于唇向倾斜,下颌第一恒磨牙趋于近中倾斜。
