低能量激光照射加速正畸牙齿移动和减轻正畸疼痛的临床研究

【摘要】:目的:本研究使用低能量激光照射正畸移动的牙齿,探讨不同能量密度的低能量激光照射对正畸患者疼痛程度和牙齿移动速度的影响,为确定适宜的照射能量密度提供临床依据。方法:选择使用AO自锁托槽固定矫治的患者24名,年龄在18-29岁之间,均拔除上颌第一前磨牙,使用微种植钉支抗内收前牙。将24名患者上颌左右两侧随机分为对照组(加力后模拟照射,n=16)、实验组1(加力后照射20s,能量密度为15.92J/cm2,n=16)、实验组2(加力后照射40s,能量密度为31.85J/cm2,n=16)。研究开始时间为排齐整平后开始拉上颌尖牙向远中移动阶段,弓丝为0.018×0.025英寸不锈钢丝,以两侧上颌磨牙区微种植钉作为支抗,使用镍钛拉簧以150g的力量拉尖牙向远中移动。实验组在加力后第0、3、7、14天分别对上颌尖牙唇侧和舌侧牙颈部、牙根中部、牙根尖区6个位点的牙槽黏膜进行激光照射,对照组模拟照射(无电源,盲法)。每4周复诊一次,重新加力,并检测力值为150g。每次复诊加力后使用低能量激光照射上颌尖牙。研究时间为4个月,于研究开始和结束时分别取患者上颌模型灌制硬石膏,测量尖牙牙尖到第一磨牙近中颊尖的距离,与第一次所得距离差值即尖牙向远中移动的距离。所有患者根据国际通用的VAS疼痛评分量表,采用调查问卷的方式记录首次尖牙加力后1-7天的疼痛反应,将正畸疼痛程度数据化。告知患者治疗期间不服用止痛药物。向患者及家长详细说明问卷的内容和要求以征得患者的配合。结果:1.在牙齿移动实验中,尖牙远中移动距离均值实验组1(4.432±0.571mm)㧐实验组2(3.724±0.320mm)㧐对照组(3.532±0.516mm),实验组1与对照组、实验组2均数差异有统计学意义(P㩳0.05),实验组2和对照组没有统计学差异(P㧐0.05),表明低能量激光照射(照射20s,能量密度为15.92J/cm2)可以加速牙齿移动。高剂量的激光照射没有加速牙齿移动。2.在牙齿疼痛实验中,疼痛总均值对照组(2.11±0.09)㧐实验组2(2.00±0.09)㧐实验组1(1.45±0.09),实验组1与对照组、实验组2均数差异有统计学意义(P㩳0.05),实验组2和对照组没有统计学差异(P㧐0.05),表明低能量激光照射(照射20s,能量密度为15.92J/cm2)可以减轻正畸牙齿的疼痛。高剂量的激光照射没有减轻正畸牙齿的疼痛。结论:低能量激光治疗可以加速正畸牙齿移动并减轻受力牙的疼痛,但需要适宜的照射剂量。照射剂量过高时不能加速正畸牙齿移动,也不能减轻受力牙的疼痛。
