上颌尖牙阻生及邻牙牙根吸收的三维空间研究

【摘要】:目的利用锥形束CT(cone beam computed tomography)在三维空间中定位上颌阻生尖牙及确定最常见阻生类型,同时评估邻牙(中切牙和侧切牙)牙根吸收情况。 方法对63例上颌阻生尖牙患者进行锥形束CT扫描,采用多平面重建(multiplanarreconstructions,MPR)获得牙体表面的三维重建图像,观察上颌阻生尖牙的位置。在数字化三维成像图中获取矢状、冠状、轴切面视图后,设定X、Y、Z平面,并对每个上颌阻生尖牙牙尖定位。同时观察上颌阻生尖牙邻近切牙的牙根吸收程度。 结果1.尖牙近中腭侧阻生占总数的56.94%(n=41),尖牙近中唇侧阻生占总数的33.33%(n=24)。在矢状面,测量上颌阻生尖牙牙尖距离X平面的距离为25.20~40.30mm,平均32.40mm;在冠状面,尖牙偏近中阻生的占90.28%(n=65),至Y平面的距离为7.50~13.60mm,平均10.80mm,尖牙偏远中阻生的占9.72%(n=7),至Y平面的距离为10.40~15.10mm,平均12.40mm;在轴切面,尖牙偏腭侧阻生的占61.11%(n=44),至Z平面的距离为4.60~10.90mm,平均8.90mm,尖牙偏唇侧阻生的占38.89%(n=28),至Z平面的距离为3.50~11.80mm,平均9.70mm。 2.上颌阻生尖牙第一次与第二次测量的数值比较后无统计学差异(P>0.05)。 3.上颌阻生尖牙导致侧切牙的牙根吸收比率为34.7%(n=25),导致中切牙的牙根吸收比率为19.4%(n=14),两者差别有统计学意义(χ2=4.26,P<0.05)。切牙的牙根吸收主要为轻度吸收和中度吸收,,轻度吸收占53.8%(n=21),中度吸收占35.9%(n=14)。 4.尖牙近中阻生导致切牙牙根吸收比率为60%,显著高于尖牙远中阻生,两者牙根吸收比率的差别有统计学意义(χ2=9.16,P=0.002,P<0.05);尖牙唇侧阻生导致切牙牙根吸收比率为32%,尖牙腭侧阻生导致切牙牙根吸收比率为68%,两者牙根吸收比率有差异(χ2=8.95,P=0.03,P<0.05)。结论1.上颌阻生尖牙以偏近中、腭侧阻生常见。 2.锥形束CT在三维空间中评估阻生尖牙位置具有良好的可靠性和可重复性。 3.上颌阻生尖牙引起的牙根吸收常见于侧切牙,并且主要为轻、中度吸收。 4.尖牙近中阻生和尖牙腭侧阻生较易导致切牙牙根吸收。
[Abstract]:Objective to locate maxillary impacted fangs and determine the most common types of impacted teeth in three-dimensional space by conical beam CT (cone beam computed tomography), and to evaluate the root resorption of adjacent teeth (middle incisors and lateral incisors). Methods 63 patients with maxillary impacted fangs were scanned by conical beam CT. The three-dimensional reconstruction images of maxillary impacted fangs were obtained by multi-plane reconstruction (multiplanarreconstructions,MPR), and the position of maxillary impacted fangs was observed. After obtaining sagittal, coronal and axial views in the digital three-dimensional imaging map, set the X, Y, Z plane, and locate the cusp of each maxillary impacted fangs.
