荆州市流感相关疾病住院病例流行病学研究

【摘要】:目的和意义对湖北省荆州市两城区医疗机构进行回顾性调查,描述该地区2006年-2008年间流感相关疾病的住院负担,分析该类疾病的流行病学特征,为制定流感的防控策略提供科学依据,同时也为即将在该地区开展的基于人群的严重急性呼吸道感染病例监测活动提供基础资料。 研究方法选定全市所有共36家医疗机构为研究现场,发放统一设计的调查问卷,对全市医疗资源进行调查;运用文献回顾法,综合文献中所述流感相关疾病定义并结合所调查地区的医师对疾病命名的习惯,将所涉及疾病的ICD-10编码细分到亚目,制定出本调查统一使用的流感相关疾病的病例定义。以此定义作为检索关键变量,查阅各医疗机构的住院病例,用统一的调查筛录表收集相关信息。使用描述性分析的方法对该地区的流感相关疾病的住院负担进行评估,同时用SPSS17.0软件对数据进行卡方检验,分析比较不同年度、不同年龄组以及不同性别病例之间住院率的差异,并通过多分类非条件Logistic回归分析对影响居民选择不同级别医疗机构治疗流感相关疾病的因素进行分析。 结果医疗资源调查发现,全市医疗机构共拥有床位6022张,年度住院总量为163300人次,其相应构成比均为综合医院最大,而社区卫生服务中心最小。其构成与拥有床位量相似。由病例回顾性调查得到,2006年1月1日~2008年12月31日间,共有27家医疗机构收治了38020例流感相关住院病例,年平均住院率为11.0‰,呈逐年上升趋势。0~1岁年龄组的住院率最高,达到335.0‰;65岁以上年龄组的病死率最高,达到1.8%。男性流感相关疾病住院率略高于女性。流感相关疾病在全年均可发生,其中3月份住院病例数最多,出现春季高峰。急性上呼吸道感染是导致当地居民住院最主要的流感相关疾病之一。通过多分类非条件Logistic回归分析可知,影响患有流感相关疾病的居民选择就诊医疗机构的因素可能包括年龄、性别以及入院年份等。 结论研究结果提示卫生决策者应该合理配置卫生资源,尤其是加强基层医疗卫生服务的建设。该地区流感相关疾病的住院负担相对较重,特别是0-1岁儿童和65岁以上老年人群,为流感相关疾病的高危人群,是流感防治的重点对象。
[Abstract]:Objective and significance of retrospective investigation of medical institutions in two urban areas of Jingzhou City, Hubei Province, to describe the hospitalization burden of influenza related diseases from 2006 to 2008, and to analyze the epidemiological characteristics of these diseases. It provides a scientific basis for the formulation of influenza prevention and control strategies and provides basic information for the upcoming population-based surveillance of severe acute respiratory infection cases in the region. Methods A total of 36 medical institutions in the city were selected as the research site and a unified questionnaire was issued to investigate the medical resources of the whole city. Using literature review method, combining the definitions of influenza-related diseases mentioned in the literature and the doctors' habit of naming diseases in the investigated areas, the ICD-10 coding of the diseases involved was subdivided into suborders. Develop a unified case definition of influenza-related diseases used in this survey. This definition is used as a key variable to retrieve the inpatient cases in various medical institutions and to collect relevant information with a uniform survey and screening table. The inpatient burden of influenza related diseases in this area was assessed by descriptive analysis method, and the data were checked by chi-square with SPSS17.0 software, and analyzed and compared in different years. The differences of hospitalization rate among different age groups and different genders were analyzed. The factors influencing residents to choose different medical institutions to treat influenza-related diseases were analyzed by multi-classification non-conditional Logistic regression analysis. Results the investigation of medical resources showed that the total number of beds in medical institutions was 6022 and the total annual hospitalization was 163300 person-times. The corresponding constitution ratio was the largest in general hospitals and the smallest in community health service centers. Its composition is similar to the number of beds owned. From the retrospective investigation of cases, from January 1, 2006 to December 31, 2008, a total of 38020 influenza-related inpatients were admitted to 27 medical institutions, with an average annual hospitalization rate of 11.0 鈥,
