血清唾液酸与类风湿关节炎相关性的研究

【摘要】:研究目的:通过测定唾液酸(SA)在类风湿关节炎(RA)患者和健康对照组血清中水平并进行对比,分析其与RA的实验室检查结果及临床表现之间的相关性,进一步探讨SA在RA发病机制中的作用及临床意义。研究方法:共收集120例RA患者和60例健康对照者。用酶法测定两组血清SA水平并比较;将RA患者血清SA与其红细胞沉降率(ESR)、C反应蛋白(CRP)、类风湿因子(RF)、抗环瓜氨酸肽抗体(抗CCP抗体)等检查结果,以及病程、治疗时间、关节肿胀计数(SJC)、关节疼痛计数(TJC)及疾病活动度评分(DAS-28)进行相关性分析。统计学分析采用t检验、U检验、卡方检验及Pearson相关检验。结果:1.RA患者的血清SA水平(864.05±167.70)明显高于健康对照组(570.99±67.37),差异具有统计学意义(P0.05)。2.RA患者血清SA水平与其CRP、ESR、RF、抗CCP抗体水平均呈正相关,但只有与CRP和ESR的相关性具有统计学意义(r=0.53,P=0.00;r=0.52,P=0.00;r=0.06,P=0.55;r=0.01,P=0.89)。3.RA患者SA水平与年龄、TJC、SJC、DAS-28(CRP)、DAS-28(ESR)、Sharp评分均呈正相关(r=0.07,P=0.43;r=0.32,P=0.00;r=0.22,P=0.02;r=0.47,P=0.00;r=0.47,P=0.00;r=0.13,P=0.58),但只有与TJC、SJC、DAS-28(CRP)、DAS-28(ESR)的相关性具有统计学意义(P0.05);与病程、治疗时间呈负相关性(r=-0.09,P=0.31;r=-0.06,P=0.51),差异无统计学意义(P0.05)。分析结果显示:1.RA的疾病活动度与血清SA水平有关。2.蛋白质糖基化末端SA的缺失可能与RA的发病有关。3.免疫球蛋白糖链的SA缺失可能诱发自身免疫反应,导致滑膜炎症从而致病。4.联合应用Ig G0、Siglec-1、RF及抗CCP抗体有助于RA的早期诊断和治疗效果的评价。5.血清SA水平可作为监测RA病情变化、指导治疗的临床指标。
[Abstract]:Objective: to determine the serum levels of sialic acid (SA) in patients with rheumatoid arthritis (RA) and healthy controls, and to analyze the correlation between sialic acid (SA) and the laboratory results and clinical manifestations of RA. To further explore the role and clinical significance of SA in the pathogenesis of RA. Methods: 120 RA patients and 60 healthy controls were collected. Serum SA levels were measured by enzyme method in two groups, and the results of serum SA and erythrocyte sedimentation rate (ESR) / C reactive protein, (CRP), rheumatoid factor (RF), anti-cyclic citrullinated peptide antibody (anti-CCP antibody), course of disease, treatment time, etc.) of RA patients were compared. Joint swelling count (SJC), joint pain count (TJC) and disease activity score (DAS-28) were analyzed. T test, chi-square test and Pearson correlation test were used in statistical analysis.
