声触诊组织量化成像技术对继发性甲状旁腺功能亢进患者的临床研究

【摘要】:目的:使用声触诊组织量化剪切波弹性成像技术(virtual touch tissue imaging quantification,VTIQ)测量维持性血液透析患者甲状旁腺组织剪切波速度(shear wave velocity,SWV)。旨在探讨VTIQ在继发性甲状旁腺功能亢进(secondary hyperparathyroidism,SHPT)中的应用价值。方法:研究组107例选自青岛大学附属医院血液透析中心慢性肾脏病(chronic kidney disease,CKD)5期血透患者,根据甲状旁腺激素(intact parathyroid hormone,i PTH)测量值将其分为3组:A组(㩳300pg/ml)、B组(300pg/ml~800pg/ml)、C组(≥800pg/ml)。正常对照组30例选自青岛大学附属医院肾病科自愿接受甲状旁腺超声及VTIQ检查医护人员。应用常规超声检查甲状旁腺,描述其二维超声图像特征。随后启用VTIQ技术测量组织的SWV,通过组间比较、相关性检验分析不同组间SWV差异及变化特征,绘制受试者工作特征曲线(receiver operator characteristic curve,ROC曲线)并寻找最佳诊断阈值。结果:通过比较组间SWV值发现,A组(2.081±0.463)m/s与正常对照组(1.978±0.303)m/s差异无统计学意义(P=0.430),B组(2.428±0.502)m/s、C组(3.022±0.707)m/s SWV均较正常对照组增大(P=0.001、0.000),A、B组,B、C组,A、C组SWV组间差异有统计学意义(P=0.005、0.000、0.000)。i PTH与SWV之间呈正相关(r=0.680,P=0.000)。以i PTH值300pg/ml和800pg/ml分别作为诊断SHPT及手术的截断点,对SWV进行ROC分析发现其曲线下面积分别为0.798、0.842(均P=0.000),截断值为2.260m/s、2.720m/s,敏感度分别为79.17%、80.00%,特异度分别为77.14%、83.33%。结论:VTIQ技术可无创定量评估继发性甲状旁腺功能亢进患者的甲状旁腺弹性变化。剪切波速度值2.260m/s和2.720m/s可以分别作为诊断SHPT及手术治疗的参考点,为评估甲状旁腺功能提供有利的诊断依据,进而做出不同临床干预具有广阔应用前景。
