中国南方地区中老年患者血清维生素D水平与骨密度的相关性

【摘要】:背景:维生素D是促进钙吸收和维持人体骨骼系统健康的重要物质,但其血清水平与人体骨密度的关系仍存在争议。目的:探索中国南方地区中老年患者血清维生素D水平与骨密度的相关性。方法:连续采集中国南方地区一个大型三甲医院于2014年7月至2016年12月就诊的50岁以上门诊或住院患者的临床数据,分析患者的血清25-羟基维生素D[25(OH)D]水平与骨密度(腰椎、股骨颈和全髋)的关系。结果与结论:(1)纳入对象资料分析:共纳入1 154例患者,年龄(64.62±10.48)岁,女性占77.2%,平均血清25(OH)D水平为(62.07±37.40)nmol/L。血清25(OH)D随着年龄增大逐渐降低,50-59岁、60-69岁和70岁以上患者25(OH)D不足/缺乏的比例分别为71.3%,77.0%和80.1%;(2)相关性分析显示,总体患者的血清25(OH)D水平与腰椎骨密度无相关性,但与股骨颈和全髋骨密度存在弱正相关性(r=0.09,0.08,P0.01)。在维生素D缺乏[25(OH)D≤50 nmol/L]患者群中,血维生素D水平与腰椎骨、股骨颈和全髋骨密度呈显著正相关(r=0.117,0.120,0.146,P0.01);(3)以性别做亚组分析,男性维生素D缺乏患者,其维生素D水平与腰椎、股骨颈和全髋骨密度都不存在相关性;女性维生素D缺乏患者,其维生素D水平与腰椎、股骨颈和全髋骨密度存在显著正相关性(r=0.105,0.135,0.171,P0.01);(4)结果证实,中国南方地区中老年患者普遍存在维生素D不足或缺乏,但男性患者血清维生素D水平与骨密度无相关,女性患者维生素D水平与骨密度正相关。
[Abstract]:Background: vitamin D is an important substance to promote calcium absorption and maintain the health of human skeletal system, but the relationship between serum level and bone mineral density is still controversial. Objective: to explore the correlation between serum vitamin D level and bone mineral density (BMD) in elderly patients in southern China. Methods: the clinical data of outpatients over 50 years old or hospitalized in a large third Class A hospital in southern China from July 2014 to December 2016 were collected. The relationship between serum 25-hydroxyvitamin D [25 (OH) D] and bone mineral density (lumbar vertebra, femoral neck and total hip) was analyzed. Results and conclusion: (1) data analysis of the subjects included: a total of 1 154 patients with age of (64.62 卤10.48) years old, female accounted for 77.2%, the average serum level of 25 (OH) D was (62.07 卤37.40) nmol/L.. The ratio of 25 (OH) D deficiency / deficiency in patients aged 50-59, 60-69 and over 70 was 71.3% and 80.1%, respectively.
