△Np63在鲍温样丘疹

【摘要】:实验目的: 1、探讨ΔNp63在正常皮肤、鲍温样丘疹病、鲍温病、皮肤鳞状细胞癌组织中的表达及意义。 2、探讨鲍温样丘疹病与鲍温病的免疫学鉴别诊断指标。 实验方法: 用免疫组化Envision法检测ΔNp63的表达,其中正常皮肤10例、鲍温样丘疹病30例、鲍温病30例、皮肤鳞状细胞癌30例。PBS代替—抗,作为阴性对照。 实验结果: 1、ΔNp63在10例正常皮肤组织的表皮基底层及表皮下1/3的细胞表达,在30例鲍温样丘疹病组织中6例为表皮内弥漫表达,24例为表皮基底层及表皮下1/3的细胞表达;在30例鲍温病组织中1例阴性表达,29例为表皮内弥漫表达;在30例皮肤鳞状细胞癌组织的表皮及真皮内癌巢的基底样细胞表达,在高分化鳞癌(21例)的癌巢外周呈环状表达,在中分化鳞癌(9例)的癌巢中有较弥漫的表达。 2、SPSS13.0统计软件进行X2检验,ΔNp63在鲍温病、皮肤鳞状细胞癌组中的表达相比较差异有统计学意义(X2=0.001,P0.05),在鲍温病组中的阳性表达率高于皮肤鳞状细胞癌组。ΔNp63在鲍温样丘疹病组和鲍温病组中表达的比较差异有统计学意义(X2=0.000,P0.05),鲍温病组中的阳性表达率高于鲍温样丘疹病组。ΔNp63在皮肤鳞状细胞癌组中与鲍温样丘疹病组中表达的比较(X2=0.125,P0.05),无统计学差异。ΔNp63在鲍温样丘疹病组与正常皮肤组中表达的比较(X2=0.072,P0.05)无统计学差异。正常皮肤组和皮肤鳞状细胞癌组中ΔNp63的表达差异有统计学意义(X2=0.001,P0.05),在皮肤鳞状细胞癌组中的表达明显高于正常皮肤组。 鲍温病和皮肤鳞状细胞癌的曝光部位构成比(曝光部位例数/总例数×100)比较(X2=0.000,P0.05)差异有统计学意义,皮肤鳞状细胞癌发病以曝光部位为主。 鲍温病中曝光部位及非曝光部位比较,ΔNp63的表达无统计学差异(X2=0.063,P0.05)。皮肤鳞状细胞癌中曝光部位与非曝光部位比较,ΔNp63的表达无统计学差异(X2=0.212,P0.05)。 3、SPSS13.0统计软件进行相关分析:ΔNp63阳性表达率与表皮中的表达模式*(P=0.000,P0.05)呈正相关,表皮中弥漫表达模式的ΔNp63阳性表达率高;ΔNp63阳性表达率与鳞状细胞癌分化程度(P=0.004,P0.05)呈正相关,分化程度低的ΔNp63阳性表达率高;ΔNp63阳性表达率与曝光部位无相关性(P=0.966,P0.05)。 病种与年龄(P=0.000,P0.05)呈正相关,随年龄增加皮肤鳞状细胞癌发病增加;病种与曝光部位(P=0.000,P0.05)有相关性,本研究调查发现鲍温样丘疹病和鲍温病发病多在非曝光部位,皮肤鳞状细胞癌发病多在曝光部位:病种与表皮中的表达模式*(P=0.000,P0.05)呈正相关,鲍温样丘疹病多呈表皮基底层及表皮下1/3的细胞表达而鲍温病呈表皮内弥漫表达;病种与阳性表达率(P=0.000,P0.05)呈正相关,病种不同阳性表达率也不同。 实验结论: 1、皮肤鳞状细胞癌多发生在曝光部位,发病与紫外线照射有一定关系。 2、ΔNp63多考虑是未分化及低分化的上皮来源肿瘤的标记。 3、ΔNp63可作为鉴别鲍温样丘疹病及鲍温病的免疫学指标。
[Abstract]:Objective:
1, to investigate the expression and significance of delta Np63 in normal skin, Bowen like papules, Bowen's disease, and skin squamous cell carcinoma.
2, to explore the immunological differential diagnosis of Bowen like papular disease and abalone fever.
Experimental methods:
Immunohistochemical Envision method was used to detect the expression of Np63, including 10 cases of normal skin, 30 cases of Bowen's papule disease, 30 cases of Bowen's disease, 30 cases of cutaneous squamous cell carcinoma,.PBS as a negative control.
Experimental results:
1, the expression of Np63 in 10 cases of normal skin tissue in the basal layer of the epidermis and epidermal 1/3 cells in 30 cases of Bowenoid papulosis tissues and 6 cases of skin diffuse expression, 24 cases of basal layer of the epidermis and epidermal 1/3 cells expression; in 30 cases of Bowen's disease in 1 cases. The expression of 29 cases of skin diffuse expression; expression of basal cells in the epidermis and dermis of 30 cases of skin squamous cell carcinoma tissue and carcinoma in the nest in the high differentiated squamous cell carcinoma (21 cases) of the cancer nest peripheral annular expression in squamous cell carcinoma (9 cases) was filled with nests.
2, SPSS13.0 statistical software for X2 test, a Np63 in Bowen's disease, a statistically significant expression of cutaneous squamous cell carcinoma group compared (X2=0.001, P0.05), the positive expression in Bowen's disease group was higher than that of cutaneous squamous cell carcinoma group. There was significant difference between the expression of Np63 in Bowenoid papulosis group and Bowen's disease in the group (X2=0.000, P0.05), the positive expression of Bowen's disease group was higher than that of Bowenoid papulosis group. Comparison of Np63 in squamous cell carcinoma group and Bowenoid papulosis group expression (X2=0.125, P0.05), no statistically significant difference. Comparison of expression of Np63 in Bowenoid papulosis group and normal skin in the group (X2=0.072, P0.05) had no significant difference. There was significant difference in the expression of normal skin and cutaneous squamous cell carcinoma group in a Np63 (X2=0.001, P0.05), the expression in cutaneous squamous cell carcinoma was significantly higher than normal Skin group.
The proportion of exposure sites of Bowen's disease and skin squamous cell carcinoma (exposure site number / total number of cases 100) was statistically significant (X2=0.000, P0.05). The incidence of cutaneous squamous cell carcinoma was mainly exposed.
There was no significant difference in the expression of delta Np63 between Bowen's disease and exposed parts. There was no significant difference in the expression of delta X2=0.063 (X2=0.063, P0.05). There was no significant difference in the expression of delta Np63 between the exposure site and the non exposed part of the skin squamous cell carcinoma (X2=0.212, P0.05).
3, SPSS13.0 statistical software for correlation analysis: the rate and patterns of expression in the epidermis. The positive expression of Np63 * (P=0.000, P0.05) were positively correlated in the epidermis of diffuse expression of delta Np63 expression pattern of high rate; a positive expression rate of Np63 and the degree of differentiation of squamous cell carcinoma (P=0.004, P0.05) was positively related to Np63 positive the expression of low degree of differentiation rate is high; a positive expression rate of Np63 and the exposure site no correlation (P=0.966, P0.05).
Disease and age (P=0.000, P0.05) was positively related to the incidence of squamous cell carcinoma of the skin increased with age; disease and exposure sites (P=0.000, P0.05) are related, the survey found that Bowenoid papulosis and Bowen's disease in non exposed parts, the incidence of skin squamous cell carcinoma in the exposure site: the expression pattern of disease and skin in * (P=0.000, P0.05) were positively correlated, Bowenoid papulosis is the expression of the basal layer of the epidermis and epidermal cells of 1/3 and Bowen's disease showed diffuse expression of epidermal diseases; and the positive expression rate (P=0.000, P0.05) were positively related to different diseases, the positive expression rate is also different.
Experimental conclusions:
1, skin squamous cell carcinoma occurs mostly at the exposure site, which has a certain relationship with ultraviolet radiation.
2, Delta Np63 is considered as a marker of undifferentiated and poorly differentiated epithelial derived tumors.
3, Delta Np63 can be used as an immunological index to differentiate abalone from papular disease and abalone fever.
