【摘要】:目的:湿疹是一种是由多种内外因素引起的变态反应性疾病,发病机制复杂,原因不明,极度瘙痒且易反复发作。以往认为在湿疹的发病中Th2占优势的Th1/Th2细胞亚群的失衡是其发病的一个主要机制,但此失衡理论并不能完全解释其发病机制。新近研究发现,机体存在一种新型的、以分泌IL-17为主要特征的细胞群,其在分化、生物学作用等方面不同于1型和2型T细胞,被命名为Th17细胞。IL-23也是近年新发现的细胞因子,与IL-12共同分享IL-12p40和IL-12Rβ1亚单位。研究发现,IL-23主要作用于记忆CD4+T细胞,诱导Th1型细胞的分化及IFN-γ产生,并发现其还可作用于记忆Th17细胞亚群诱导产生IL-17,对于Th17细胞的生存和活性有重要的辅助作用。因此Th17细胞亚群和IL-17、IL-23的发现为人们重新认识免疫应答和调节机制提供了方向。本实验主要研究IL-17和IL-23在湿疹患者和正正常人外周血中的表达及其差异,初步探讨1L-17和IL-23在湿疹发病机制中的作用和临床意义。 方法:参照赵辩主编的临床皮肤病学诊断标准和相关文献选取45例湿疹患者,男23例,女22例,年龄16—78岁,平均(35.68±10.58)岁。按照临床表现将患者进行分期,其中急性期患者15例,亚急性期15例,慢性期15例。以性别和年龄相匹配的30例健康体检者(男14例,女16例,年龄18—76岁,平均37.22±8.56岁)作为对照组。采用ELISA方法检测血清中IL-17和IL-23的表达水平,并收集整理湿疹患者的临床资料及实验室数据。 结果: 1.急性期和亚急性期湿疹患者血清中IL-17水平明显高于慢性期患者和正正常组(p0.05),但IL-17水平在急性期和亚急性期、慢性期和正正常组间则无统计学差异。 2.急性期和亚急性期湿疹患者血清中IL-23水平明显高于慢性期患者和正正常组(p0.05),但IL-23水平在急性期和亚急性期、慢性期和正正常组间则无统计学差异。 3.湿疹患者血清中1L-17与IL-23表达水平呈正直线相关(r=0.53,P0.05)关系。 4.湿疹患者血清中IL-17和IL-23水平与年龄、血沉、白蛋白、球蛋白、血肌酐、淋巴细胞、嗜酸性粒细胞计数和血小板计数之间无明显相关关系。 结论:IL-17和IL-23在湿疹患者血清中表达水平增高,且急性期和亚急性期更为明显,具有统计学差异。提示IL-17和IL-23可能参与了湿疹疾病的病理过程,并且与患者疾病的严重程度有关系。
