光动力疗法治疗BCC及SCC的临床疗效观察

【摘要】:目的探讨光动力疗法(5-aminolevulinic acid-photodynamic therapy,ALA-PDT)治疗BCC及SCC临床疗效、安全性及复发率。 方法临床治疗前后疗效观察,对于癌肿较大或浸润较深者每次治疗前先行液氮冷冻,疗程为4-8周。末次治疗后进行疗效评价,分别于治疗后3、6、12和24月观察复发率,同时观察不良反应。 结果36例患者中19例完全有效,治愈率为52.8%,包括14例浅表型BCC和5例结节型BCC。部分缓解为25.0%,总有效率为86.1%,无效率为13.9%。仍在随访中,目前暂未发现复发患者。 结论外用ALA-PDT方法治疗BCC及SCC疗效存在差异。对浅表型BCC治愈率高,结节型BCC有效,但治愈率偏低,建议颜面等特殊部位且较小者,有美容要求或不能耐受手术的患者临床上可作为首选治疗方案。对于较大肿瘤不易手术切除患者可作为术前准备治疗。但对SCC疗效欠佳,尚需进一步临床观察疗效。
[Abstract]:Objective to investigate the efficacy, safety and recurrence rate of 5-aminolevulinic acid-photodynamic therapy in the treatment of BCC and SCC. Methods the therapeutic effect before and after treatment was observed. The patients with large cancer or deep infiltration were frozen with liquid nitrogen before each treatment. The course of treatment was 4-8 weeks. The curative effect was evaluated after the last treatment. The recurrence rate was observed at 12 and 24 months after treatment and the adverse reactions were observed at the same time. Results among the 36 patients, 19 cases were completely effective, the cure rate was 52.8%, including 14 cases of superficial BCC and 5 cases of nodular BCC. The total effective rate was 86.1, and the inefficiency was 13.9. Still in follow-up, at present, no recurrence has been found. Conclusion there are differences in the efficacy of ALA-PDT in the treatment of BCC and SCC. The cure rate of superficial BCC is high, the nodular BCC is effective, but the cure rate is low. It is suggested that the patients with special position and small face, who have cosmetic requirements or can not tolerate surgery can be regarded as the first choice in clinic. For patients with large tumors who are not easily resected surgically, it can be used as preparation for treatment before operation. However, the efficacy of SCC is not good, and further clinical observation is needed.
