110例手部湿疹斑贴试验临床分析

【摘要】:目的:探讨手部湿疹的潜在病因及该类患者对日常接触致敏原的敏感性,并分析其性别、年龄和职业特点。 方法:研究对象为2008年7月至2009年4月间在广州医学院第二附属医院皮肤科就诊的手部慢性湿疹患者共110例,所有入选患者15天内未使用过糖皮质激素,7天内未使用过免疫调节剂,3天内未服用过抗组胺药,并排除湿疹处于急性期的患者以及孕妇。应用“瑞敏”斑贴试剂盒检测患者对日常接触致敏原的敏感性,结果按阴性、可疑阳性、弱阳性、强阳性、极强阳性、刺激性反应分类分级判定。分别按性别、年龄、职业分组,分析各种过敏原的总体阳性率及在不同组别中的阳性率。采用SPSS 13.0统计软件,用χ2检验分析阳性率差异的显著性,P0.05为差异有统计学意义。 结果:受检的110例患者中有94例(85.5%)斑贴试验出现阳性反应,位列前5种最常见的致敏原分别为重铬酸钾、硫酸镍、氯化钴、芳香混合物和甲醛,男性患者中对苯二胺基质、秋兰姆混合物及黑橡胶混合物的阳性检出率明显高于女性。不同年龄段各种致敏原阳性率差异无显著性意义。从事某些特定职业的患者(工人、家庭主妇、布匹销售员等)阳性率明显较高。 结论:斑贴试验有助于明确手部湿疹患者的接触性致敏原,对提高其治疗的针对性,并通过避免或去除致敏原切断过敏反应,达到治愈并防止复发甚至根治的目的,有极其重要的临床意义。
[Abstract]:Objective: to investigate the potential etiology of hand eczema and its sensitivity to daily exposure allergens, and to analyze its sex, age and occupational characteristics. Methods: from July 2008 to April 2009, 110 patients with chronic eczema of hand were treated in Department of Dermatology, second affiliated Hospital of Guangzhou Medical College, all of whom had not used glucocorticoid in 15 days. No immunomodulators were used in 7 days, no antihistamines were taken in 3 days, and eczema patients and pregnant women were excluded. The sensitivity of the patients to daily contact allergens was determined by using the "Ruimin" patch kit. The results were classified according to negative, suspicious positive, weak positive, strong positive, very strong positive and irritating reaction classification. The total positive rate of each allergen and the positive rate in different groups were analyzed according to sex, age and occupation. Using SPSS 13.0 statistical software, 蠂 2 test was used to analyze the significance of positive rate, P0.05 as the difference was statistically significant. Results: 94 (85.5%) of 110 patients were positive for patch test. The most common allergens were potassium dichromate, nickel sulfate, cobalt chloride, aromatic mixture and formaldehyde. The positive rates of p-phenylenediamine matrix, chylum mixture and black rubber mixture in male patients were significantly higher than those in women. There was no significant difference in the positive rate of allergens between different age groups. The positive rate of patients (workers, housewives, cloth salesmen, etc.) in certain occupations was significantly higher. Conclusion: patch test is helpful to identify contact allergen in patients with hand eczema, to improve the pertinence of treatment, and to cure and prevent recurrence or even radical cure by avoiding or removing allergen. Has extremely important clinical significance.
